# Miconazole vaginal pessary
## Overview
Miconazole is an imidazole antifungal agent. Vaginal pessaries are intended for local treatment of vaginal candidiasis.
## Primary Indications
Treatment of vulvovaginal candidiasis (yeast infections).
## Adult Dosing
* **100 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 7 consecutive nights.
* **200 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 3 consecutive nights.
* **150 mg vaginal pessary:** Insert one pessary vaginally at bedtime for 3 consecutive nights.
* **Single dose 1200 mg vaginal suppository:** Insert one suppository vaginally as a single dose. This is available as a different formulation but is sometimes considered for single-dose therapy.
Dosing may vary based on product availability and local formulary or treatment guidelines.
## Pediatric Dosing
* **100 mg vaginal pessary:** Generally not recommended for pediatric use due to lack of established safety and efficacy data.
## Dose Adjustments
No dose adjustments are typically necessary for hepatic or renal impairment as systemic absorption is minimal.
## Contraindications
* Hypersensitivity to miconazole or any component of the formulation.
* Concurrent use with certain CYP3A4 inhibitors (see Key Drug Interactions).
## Adverse Effects
* **Common:** Vaginal burning, itching, irritation, redness, cramping.
* **Less Common:** Headache, rash.
## Key Drug Interactions
* **CYP3A4 Inhibitors:** Miconazole is a potent inhibitor of CYP3A4. Concomitant use with drugs metabolized by CYP3A4 (e.g., certain statins, benzodiazepines, calcium channel blockers, anticoagulants like warfarin) can lead to increased plasma concentrations of these drugs, potentially causing toxicity. Co-administration with oral miconazole is contraindicated; topical vaginal formulations generally have less systemic absorption, but caution is advised, particularly with prolonged or widespread use. ## Monitoring
* Monitor for symptom resolution.
* Assess for signs of local irritation or hypersensitivity.
## Clinical Pearls
* Treatment duration is typically 3 or 7 days, depending on the product strength. A single-dose option may be available.
* Advise patients to complete the full course of treatment, even if symptoms improve.
* Avoid intercourse during treatment to prevent reinfection or partner transmission.
* Latex condoms and diaphragms may be damaged by the pessary base, reducing their effectiveness. Patients should use alternative methods of contraception during treatment and for at least 72 hours after completion.
* If symptoms do not improve after the full course or if they worsen, discontinue use and consult a healthcare provider. Recurrent infections may require longer or different treatment regimens.
