# Megestrol Acetate
## Overview
Megestrol acetate is a synthetic progestin with antiandrogenic and antineoplastic activity. It is primarily used for palliative treatment of advanced breast cancer and in cachexia associated with acquired immunodeficiency syndrome (AIDS).
## Primary Indications
* Palliative treatment of advanced, recurrent, or metastatic breast cancer in postmenopausal women.
* Treatment of cachexia, anorexia, or unexplained significant weight loss in patients with acquired immunodeficiency syndrome (AIDS).
## Adult Dosing
* **Breast Cancer:** 40 mg orally once daily. Higher doses may be used.
* **AIDS Cachexia:** 800 mg orally once daily.
## Pediatric Dosing
There is no established pediatric dosing for megestrol acetate.
## Dose Adjustments
* **Renal Impairment:** No specific dose adjustment is recommended.
* **Hepatic Impairment:** No specific dose adjustment is recommended, but caution is advised.
## Contraindications
* Known or suspected pregnancy.

* **Aminoglutethimide:** May decrease serum concentrations of megestrol acetate.
## Monitoring
* **Breast Cancer:** Monitor for tumor response, signs of disease progression, and adverse effects.
* **AIDS Cachexia:** Monitor for weight gain, appetite, and adverse effects.
* **General:** Monitor for signs and symptoms of thromboembolic events, adrenal insufficiency, and Cushingoid effects. Blood pressure should be monitored regularly.
## Clinical Pearls
* Megestrol acetate can cause significant weight gain, primarily due to increased appetite and fluid retention.
* Abrupt discontinuation after prolonged use may lead to adrenal insufficiency. Tapering the dose is recommended.
* Use with caution in patients with a history of thromboembolic disease.
* Effective contraception should be used by patients of childbearing potential.
