## Overview
Mega%25252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252520cv is a potent anticoagulant agent.
## Primary Indications
* Prophylaxis and treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE).
* Prevention of stroke and systemic embolism in patients with non-valvular atrial fibrillation.
* Reduction in the risk of myocardial infarction, stroke, and death in patients with acute coronary syndrome or recent myocardial infarction.
## Adult Dosing
Dosing varies significantly based on indication and renal function. Specific protocols are often followed.
* **DVT/PE Prophylaxis:** 2.5 mg orally once daily.
* **DVT/PE Treatment:** 15 mg orally once daily for 7 days, followed by 5 mg orally once daily. The duration of therapy should be individualized.
* **Non-valvular Atrial Fibrillation Stroke Prevention:** 5 mg orally once daily. In patients with reduced renal function (CrCl 15-50 mL/min), the dose is 2.5 mg orally once daily.
* **Acute Coronary Syndrome (ACS):** 2.5 mg orally once daily, in addition to aspirin. For patients with ACS and a history of DVT or PE, the dose may be increased to 5 mg orally twice daily if bleeding risk is low.
Maximum dose generally ranges from 10 mg to 30 mg daily depending on the indication.
## Pediatric Dosing
There is no established pediatric dosing for Mega%25252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252520cv. Use in pediatric populations is not recommended.
## Dose Adjustments
* **Renal Impairment:** Dose reduction is required in moderate to severe renal impairment (CrCl 15-50 mL/min). Specific reductions depend on the indication. Generally, avoid in severe renal impairment (CrCl <15 mL/min).
* **Hepatic Impairment:** Caution is advised. No specific dose adjustments are typically provided, but caution is warranted due to increased bleeding risk.
## Contraindications
* Active serious bleeding.
* Hypersensitivity to Mega%25252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252520cv or any of its excipients.
## Adverse Effects
The most significant adverse effect is bleeding, which can be serious or fatal. Other adverse effects include:
* Nausea, vomiting, diarrhea
* Abdominal pain
* Dizziness, headache
* Increased liver enzymes
* Anemia
## Key Drug Interactions
* **Strong CYP3A4 inhibitors/inducers:** Can alter Mega%25252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252520cv concentrations.
* **Other anticoagulants (e.g., warfarin, heparin, DOACs):** Increased risk of bleeding.
* **Antiplatelet agents (e.g., aspirin, clopidogrel):** Increased risk of bleeding.
* **NSAIDs:** Increased risk of gastrointestinal bleeding.
## Monitoring
* Clinical assessment for signs and symptoms of bleeding.
* Regular monitoring of renal function.
* While routine coagulation monitoring is generally not required, it may be considered in specific situations (e.g., significant bleeding, planned surgery, extremes of weight, renal or hepatic impairment).

* Mega%25252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252525252520cv offers predictable pharmacokinetics and does not typically require routine coagulation monitoring, unlike warfarin.
* Ensure patients are educated on signs of bleeding and when to seek medical attention.
* Consider the indication and patient-specific factors (renal function, bleeding risk, concomitant medications) when selecting the appropriate dose.
