# Megestrol Acetate
## Overview
Megestrol acetate is a synthetic progestogen derivative. It is available in oral suspension and tablet forms.
## Primary Indications
* **Cancer:** Treatment of advanced endometrial carcinoma and breast carcinoma.
* **Appetite Stimulation:** Management of anorexia, cachexia, or unexplained weight loss in patients with acquired immunodeficiency syndrome (AIDS) or cancer.
## Adult Dosing
* **Endometrial Carcinoma:** 40 mg orally 4 times daily. Doses up to 400 mg daily have been used.
* **Breast Carcinoma:** 40 mg orally 4 times daily.
* **Appetite Stimulation:** 400-800 mg orally daily.
## Pediatric Dosing
There is no established pediatric dosing for megestrol acetate. Its use in pediatric patients is generally not recommended due to limited data and potential hormonal effects.
## Dose Adjustments
No specific dose adjustments are routinely required for hepatic or renal impairment, but caution is advised, and clinical monitoring is essential.
## Contraindications
* Known or suspected pregnancy.
* Known or suspected hypersensitivity to megestrol acetate.
* Use for diagnosis of pregnancy.
## Adverse Effects
Common adverse effects include weight gain, increased appetite, nausea, vomiting, diarrhea, hot flashes, edema, dyspnea, fatigue, and hypertension. More serious effects may include thromboembolic events (e.g., deep vein thrombosis, pulmonary embolism), adrenal insufficiency, and Cushingoid effects.
## Key Drug Interactions
* **CYP3A4 Inhibitors/Inducers:** May alter megestrol acetate concentrations. Monitor for efficacy and toxicity.
* **Aminoglutethimide:** May decrease serum concentrations of megestrol acetate.
## Monitoring
* Monitor for appetite stimulation and weight changes.
* Monitor for signs and symptoms of thromboembolic events.
* Monitor for adrenal insufficiency, especially with prolonged use or abrupt discontinuation.
* Regularly assess for other adverse effects such as hypertension, edema, and mood changes.
## Clinical Pearls
* Megestrol acetate can cause significant weight gain and increased appetite.
* Use cautiously in patients with a history of thromboembolic disease.
* Abrupt discontinuation may lead to adrenal insufficiency. Tapering the dose may be necessary in some patients, especially those on long-term therapy.
* Due to potential teratogenicity, effective contraception is crucial for women of childbearing potential.
