# Mega CV
## Overview
Mega CV is a combination medication containing aspirin and atorvastatin. Aspirin is an antiplatelet agent, and atorvastatin is a statin that lowers cholesterol.
## Primary Indications
* Prevention of cardiovascular events (e.g., myocardial infarction, stroke) in patients with established cardiovascular disease or multiple risk factors.
## Adult Dosing
* **Aspirin:** Typically 75 mg to 100 mg orally once daily.
* **Atorvastatin:** Dosing varies based on patient's cardiovascular risk and cholesterol levels. Common starting doses are 10 mg to 20 mg orally once daily. Doses can range from 10 mg to 80 mg orally once daily.
## Pediatric Dosing
* The use of Mega CV is generally not recommended in pediatric patients due to lack of established safety and efficacy data for the combination product. Aspirin use in children with viral infections carries a risk of Reye's syndrome.
## Dose Adjustments
* **Renal Impairment:** No dose adjustment is typically required for atorvastatin. Aspirin dose should be individualized, and caution is advised in severe renal impairment.
* **Hepatic Impairment:** Atorvastatin is contraindicated in active liver disease. Caution is advised in patients with mild to moderate hepatic impairment. Aspirin should be used with caution in patients with liver disease.

* **CYP3A4 Inhibitors/Inducers:** Increased atorvastatin levels with inhibitors (e.g., clarithromycin, ritonavir, itraconazole), decreased levels with inducers (e.g., rifampin).
* **Other Antiplatelets/Anticoagulants:** Increased risk of bleeding (e.g., warfarin, clopidogrel, heparin).
* **NSAIDs:** Increased risk of gastrointestinal bleeding with aspirin.
* **Gemfibrozil, Fenofibrate:** Increased risk of myopathy with atorvastatin.
## Monitoring
* Liver function tests (ALT, AST) at baseline and periodically thereafter.
* Creatine kinase (CK) levels if myopathy is suspected.
* Signs and symptoms of bleeding.
* Lipid panel as appropriate to assess efficacy.
