# Mega-CV
## Overview
Mega-CV is a combination antibiotic containing amoxicillin and clavulanate potassium. Amoxicillin is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis. Clavulanate potassium is a beta-lactamase inhibitor that protects amoxicillin from degradation by certain bacterial beta-lactamases, thereby extending its spectrum of activity.
## Primary Indications
* Community-acquired pneumonia
* Acute bacterial sinusitis
* Acute otitis media
* Skin and soft tissue infections (including cellulitis, animal bites, and periapical abscesses)
* Urinary tract infections (when resistant organisms are suspected or confirmed)
## Adult Dosing
* **Standard Dosing:** 500 mg amoxicillin/125 mg clavulanate orally every 8 hours for 7-10 days.
* **Higher Dosing (for more severe infections or those caused by less susceptible organisms):** 875 mg amoxicillin/125 mg clavulanate orally every 12 hours for 7-10 days.
## Pediatric Dosing
* Dosing is based on amoxicillin component and the severity of infection.
* **For mild to moderate infections:** 25 mg/kg/day amoxicillin (as Mega-CV 250mg/31.25mg or 400mg/57mg suspension) divided every 8 hours, or 45 mg/kg/day (as Mega-CV 200mg/28.5mg suspension) divided every 12 hours.
* **For severe infections:** 45 mg/kg/day amoxicillin (as Mega-CV 250mg/31.25mg or 400mg/57mg suspension) divided every 8 hours, or 90 mg/kg/day (as Mega-CV 200mg/28.5mg suspension) divided every 12 hours.
* **Maximum dose:** Does not typically exceed 4000 mg amoxicillin/570 mg clavulanate per day.

## Dose Adjustments
* **Renal Impairment (Adults):**
* CrCl > 30 mL/min: No adjustment necessary.
* CrCl 10-30 mL/min: 500 mg amoxicillin/125 mg clavulanate every 12 hours. (For 875 mg/125 mg formulation, use 500 mg/125 mg every 12 hours).
* CrCl < 10 mL/min: 500 mg amoxicillin/125 mg clavulanate every 24 hours. (For 875 mg/125 mg formulation, use 500 mg/125 mg every 24 hours).
* **Renal Impairment (Pediatrics):** Dosing adjustments are necessary based on renal function, similar to adult guidelines. Consult specific pediatric dosing guidelines for renal impairment.
* **Hepatic Impairment:** Use with caution. Monitor hepatic function regularly.
## Contraindications
* Hypersensitivity to amoxicillin, clavulanate potassium, penicillin, or any component of the formulation.
* History of jaundice or hepatic dysfunction associated with amoxicillin/clavulanate therapy.
## Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, rash, headache, indigestion.
* **Serious:**
* Severe allergic reactions (anaphylaxis)
* Clostridioides difficile-associated diarrhea (CDAD)
* Hepatotoxicity (cholestatic jaundice, hepatitis)
* Seizures (rare, particularly with high doses or renal impairment)
* Stevens-Johnson syndrome and toxic epidermal necrolysis
## Key Drug Interactions
* **Probenecid:** May increase amoxicillin levels by decreasing renal tubular secretion.
* **Oral Contraceptives:** May decrease efficacy. Advise patients to use an alternative or additional non-hormonal method of contraception.
* **Warfarin:** May increase INR. Monitor INR closely.
* **Methotrexate:** Amoxicillin may decrease methotrexate clearance, leading to increased toxicity. Monitor methotrexate levels.
## Monitoring
* **Clinical Efficacy:** Assess for improvement in signs and symptoms of infection.
* **Hepatic Function:** Monitor liver enzymes, especially in patients with hepatic impairment or those on prolonged therapy.
* **Renal Function:** Monitor renal function, particularly in patients with pre-existing renal disease or on concomitant nephrotoxic agents.
* **Signs of Superinfection:** Monitor for fungal or bacterial superinfections.
* **Stool for C. difficile:** If diarrhea develops, particularly severe or persistent.
## Clinical Pearls
* Administer Mega-CV at the beginning of a meal to improve tolerability and absorption.
* Extended-release formulations (e.g., 1000 mg/62.5 mg) are dosed every 12 hours and should not be crushed or chewed.
* The spectrum of activity includes many common respiratory pathogens and Gram-negative bacteria like *Haemophilus influenzae*, *Moraxella catarrhalis*, and *Escherichia coli*.
* Consider local resistance patterns when selecting antibiotic therapy.
