# Mega-CV
## Overview
Mega-CV is a combination antibiotic product containing amoxicillin and clavulanate potassium. Amoxicillin is a beta-lactam antibiotic that inhibits bacterial cell wall synthesis. Clavulanate potassium is a beta-lactamase inhibitor that protects amoxicillin from degradation by certain bacterial beta-lactamases, thereby extending its spectrum of activity.
## Primary Indications
* Bacterial infections susceptible to amoxicillin, including:
* Acute otitis media
* Acute bacterial sinusitis
* Community-acquired pneumonia
* Acute exacerbations of chronic bronchitis
* Skin and skin-structure infections
* Urinary tract infections (when caused by susceptible organisms)
## Adult Dosing
Dosing is based on the amoxicillin component. Common dosages include:
* 250 mg amoxicillin/125 mg clavulanate orally every 8 hours or 500 mg amoxicillin/125 mg clavulanate orally every 8 hours.
* 875 mg amoxicillin/125 mg clavulanate orally every 12 hours.
The maximum recommended daily dose of amoxicillin is 4000 mg. The maximum recommended daily dose of clavulanate potassium is 1000 mg.
## Pediatric Dosing
Dosing is typically based on amoxicillin weight-based recommendations, usually ranging from 25 mg/kg/day to 45 mg/kg/day divided into two or three doses, depending on the severity and type of infection. The clavulanate component is dosed accordingly (e.g., 3.6 mg/kg/day to 10 mg/kg/day divided into two or three doses).
For acute otitis media, sinusitis, and lower respiratory tract infections, a common dose is 45 mg/kg/day of amoxicillin, given as Mega-CV 400 mg/57 mg per 5 mL suspension, every 12 hours.
The maximum dose for children should not exceed the maximum adult dose.
## Dose Adjustments
* **Renal Impairment:** Dose adjustment is necessary for adults and children with significant renal impairment (CrCl < 30 mL/min). Refer to specific prescribing information for detailed recommendations. For CrCl between 10-30 mL/min, reduce the frequency of dosing. For CrCl < 10 mL/min, reduce dose and frequency.
## Contraindications
* Hypersensitivity to amoxicillin, clavulanate potassium, penicillins, or any component of the formulation.
* History of jaundice or hepatic dysfunction associated with amoxicillin-clavulanate use.
## Adverse Effects
* **Common:** Diarrhea, nausea, vomiting, rash, urticaria, pruritus.
* **Serious:** Severe allergic reactions (anaphylaxis), Clostridioides difficile-associated diarrhea (CDAD), hepatotoxicity, Stevens-Johnson syndrome, toxic epidermal necrolysis.
## Key Drug Interactions
* **Probenecid:** May increase and prolong amoxicillin blood levels.
* **Oral Contraceptives:** May decrease efficacy.
* **Warfarin:** May increase INR. Monitor INR closely.
* **Allopurinol:** May increase the incidence of rash.
## Monitoring
* Monitor for signs and symptoms of hypersensitivity reactions.
* Assess for diarrhea and signs of CDAD.
* Monitor liver function tests if hepatotoxicity is suspected.
* Monitor prothrombin time/INR in patients taking warfarin.
## Clinical Pearls
* Mega-CV should be taken with or without food, but taking it with food may reduce gastrointestinal intolerance.
* Ensure adequate hydration during treatment.
* Complete the full course of therapy as prescribed, even if symptoms improve.
* Consider the potential for cross-reactivity with other beta-lactam antibiotics.
