## Megestrol Acetate (Megace)
### Overview
Megestrol acetate is a synthetic progestogen used in the management of various conditions, including cachexia, anorexia, and certain types of cancer.
### Primary Indications
* Treatment of cachexia, anorexia, or unexplained significant weight loss in patients with acquired immunodeficiency syndrome (AIDS).
* Palliative treatment of advanced breast cancer.
* Palliative treatment of advanced endometrial carcinoma.
### Adult Dosing
* **Cachexia/Anorexia (AIDS):** Typically initiated at 800 mg (20 mL) orally once daily. Higher doses of up to 1600 mg (40 mL) daily have been used.
* **Breast Cancer:** Recommended dose is 160 mg (10 mL) orally once daily.
* **Endometrial Carcinoma:** Recommended dose is 40 mg (5 mL) orally 4 times daily. Alternatively, 160 mg (10 mL) orally once daily.
### Pediatric Dosing
There is no established pediatric dosing for megestrol acetate. Use in pediatric patients is generally not recommended due to lack of safety and efficacy data.
### Dose Adjustments
* **Renal Impairment:** No specific dose adjustments are established. Use with caution.
* **Hepatic Impairment:** No specific dose adjustments are established. Use with caution.
### Contraindications
* Known or suspected pregnancy.
* Known hypersensitivity to megestrol acetate or any component of the formulation.
### Adverse Effects
Common adverse effects include weight gain, increased appetite, hot flashes, hypertension, dyspnea, nausea, vomiting, diarrhea, rash, alopecia, and headache. More serious adverse effects can include thromboembolic events (deep vein thrombosis, pulmonary embolism), adrenal insufficiency, hyperglycemia, and tumor flare.
### Key Drug Interactions
* **Aminoglutethimide:** May decrease plasma concentrations of megestrol acetate.
* **CYP3A4 Inhibitors/Inducers:** May alter megestrol acetate plasma concentrations. Monitor for efficacy and adverse effects.
### Monitoring
* **Weight and appetite:** Assess for effectiveness.
* **Blood pressure:** Monitor for hypertension.
* **Glucose levels:** Monitor for hyperglycemia, particularly in diabetic patients.
* **Signs of thromboembolism:** Educate patients on symptoms of DVT and PE.
* **Adrenal function:** Consider monitoring, especially with prolonged use or in situations of stress or illness.
### Clinical Pearls
* Megestrol acetate can cause significant weight gain and increased appetite, which may be desirable in cachectic patients.
* Women of childbearing potential should use effective non-hormonal contraception during treatment.
* Oral suspension formulations are available, which may be easier to administer for some patients.
* Adrenal insufficiency has been reported with abrupt discontinuation after prolonged therapy. Tapering the dose may be necessary.
