# MegaCV
## Overview
MegaCV is a combination medication containing a statin and ezetimibe, used to lower cholesterol levels.
## Primary Indications
* Primary hyperlipidemia (heterozygous familial and non-familial)
* Homozygous familial hypercholesterolemia (HoFH)
## Adult Dosing
* **Primary hyperlipidemia:** 10/10 mg to 10/80 mg once daily.
* **HoFH:** 10/80 mg once daily.
The usual starting dose is 10/10 mg once daily. Doses should be individualized based on patient response and goals.
## Pediatric Dosing
The safety and efficacy of MegaCV in pediatric patients have not been established.
## Dose Adjustments
* **Renal impairment:** No dose adjustment required for mild to moderate renal impairment. Use with caution in severe renal impairment.
* **Hepatic impairment:** Contraindicated in active liver disease or unexplained persistent elevations of serum transaminases.
## Contraindications
* Active liver disease or unexplained persistent elevations of serum transaminases.
* Hypersensitivity to any component of the drug.
* Concomitant use with cyclosporine (unless used for organ transplant).
## Adverse Effects
* **Common:** Diarrhea, upper respiratory tract infection, arthralgia, fatigue, cough, muscle pain, nausea, back pain, headache.
* **Serious:** Myopathy and rhabdomyolysis (especially at higher doses or when combined with other medications that can cause myopathy), hepatotoxicity, pancreatitis, angioedema, Stevens-Johnson syndrome, peripheral neuropathy.
## Key Drug Interactions
* **Cyclosporine:** Increased systemic exposure to simvastatin and ezetimibe.
* **Fibrates and Gemfibrozil:** Increased risk of myopathy.
* **Niacin:** Increased risk of myopathy.
* **Colchicine:** Increased risk of myopathy.
* **CYP3A4 inhibitors (e.g., itraconazole, ketoconazole, posaconazole, voriconazole, protease inhibitors, erythromycin, clarithromycin, telithromycin, nefazodone, cobicistat-containing products):** Increased risk of myopathy.
* **Certain calcium channel blockers (e.g., diltiazem, verapamil):** Increased simvastatin exposure.
* **Digoxin:** Increased digoxin levels may occur.
## Monitoring
* Lipid profiles (total cholesterol, LDL-C, HDL-C, triglycerides) at baseline and periodically thereafter.
* Liver function tests (ALT, AST) at baseline and as clinically indicated.
* Creatine kinase (CK) and urine myoglobin if myopathy is suspected.
## Clinical Pearls
* MegaCV can be taken with or without food.
* Higher doses of the simvastatin component (40 mg and 80 mg) are associated with an increased risk of myopathy.
* Monitor for signs and symptoms of myopathy, including muscle pain, tenderness, or weakness, especially if accompanied by fever or malaise.
* Consider risks and benefits of concomitant use with other medications known to cause myopathy.
