# Megestrol Acetate
## Overview
Megestrol acetate is a synthetic derivative of progesterone. It is a potent progestin with antineoplastic and antiestrogenic effects.
## Primary Indications
* **Cachexia, anorexia, or unexplained significant weight loss:** Associated with acquired immunodeficiency syndrome (AIDS) or cancer.
* **Endometrial carcinoma:** Advanced disease, often in combination with other treatments.
## Adult Dosing
* **Weight loss/Cachexia:**
* Initial: 800 mg orally once daily.
* Some studies have used lower doses (e.g., 400 mg/day). Dosing may be adjusted based on patient response and tolerance.
* **Endometrial carcinoma:**
* 40 mg orally four times daily (160 mg/day).
* Higher doses of up to 800 mg/day may be used.
## Pediatric Dosing
There is no established pediatric dosing for megestrol acetate. Its use in pediatric populations is generally not recommended due to limited data and potential safety concerns.
## Dose Adjustments
* **Hepatic Impairment:** No specific guidelines, but caution is advised due to hepatic metabolism.

* **CYP3A4 Inhibitors (e.g., ketoconazole, itraconazole):** May increase plasma concentrations of megestrol acetate. Use with caution and monitor for adverse effects.
* **Corticosteroids:** Concurrent use may increase the risk of adverse effects.
## Monitoring
* **Weight and appetite:** Regularly assess for effectiveness.
* **Blood pressure:** Monitor for hypertension.
* **Signs and symptoms of thromboembolic events:** Educate patients to report chest pain, shortness of breath, leg swelling, or severe headache.
