## Meftal-Spas
### Overview
Meftal-Spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and dicyclomine hydrochloride, an antispasmodic.
### Primary Indications
* Symptomatic relief of mild to moderate pain and associated spasms, particularly in gynecological conditions and gastrointestinal disorders.
### Adult Dosing
* **Meftal-Spas Tablets:** Typically one tablet (250 mg mefenamic acid / 20 mg dicyclomine HCl) orally every 8 hours as needed for pain and spasms.
* **Maximum Dosing:** Do not exceed 3 tablets in 24 hours. The total daily dose of mefenamic acid should not exceed 1500 mg, and the total daily dose of dicyclomine HCl should not exceed 120 mg.
### Pediatric Dosing
* Information for pediatric use is limited and generally not recommended due to safety concerns with NSAIDs and anticholinergics in children. Consult specific pediatric guidelines if deemed absolutely necessary, though it is often avoided.
### Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid can exacerbate renal impairment. Dose reduction may be considered, but specific guidelines are lacking. Avoid in severe renal impairment.
* **Hepatic Impairment:** Use with caution. Mefenamic acid can cause hepatic dysfunction. Avoid in severe hepatic impairment.
* **Elderly:** Increased risk of adverse effects, particularly gastrointestinal bleeding, renal dysfunction, and cardiovascular events. Use with caution and consider the lowest effective dose.
### Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, other NSAIDs, or any component of the formulation.
* History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Active or history of recurrent peptic ulcer or gastrointestinal bleeding.
* Severe heart failure, renal failure, or hepatic failure.
* Conditions where anticholinergics are contraindicated (e.g., glaucoma, prostatic hypertrophy, obstructive uropathy, obstructive gastrointestinal tract disease, myasthenia gravis, uncontrolled asthma).
* Children under 12 years of age (due to NSAID contraindications).
* Third trimester of pregnancy.
### Adverse Effects
* **Mefenamic Acid:** Gastrointestinal (nausea, vomiting, diarrhea, abdominal pain, GI bleeding, ulceration), renal (acute kidney injury, interstitial nephritis), hepatic (elevated liver enzymes, hepatitis), cardiovascular (MI, stroke), hypersensitivity (rash, bronchospasm, anaphylaxis), central nervous system (headache, dizziness, drowsiness, confusion).
* **Dicyclomine HCl:** Anticholinergic effects (dry mouth, blurred vision, urinary retention, constipation, tachycardia, drowsiness, confusion), dizziness, weakness.
### Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of GI ulceration and bleeding.
* **Other NSAIDs, Aspirin:** Increased risk of GI toxicity and additive effects.
* **ACE Inhibitors, ARBs, Diuretics:** Reduced antihypertensive effect and increased risk of renal impairment.
* **Lithium, Methotrexate:** Increased serum levels and toxicity.
* **SSRIs:** Increased risk of GI bleeding.
* **Anticholinergics:** Additive anticholinergic effects.
* **Proton Pump Inhibitors (PPIs):** May reduce the efficacy of dicyclomine.
### Monitoring
* **GI Bleeding:** Monitor for signs and symptoms (e.g., melena, hematemesis).
* **Renal Function:** Baseline and periodic monitoring of serum creatinine and BUN, especially in patients with pre-existing renal disease or those at risk.
* **Hepatic Function:** Baseline and periodic monitoring of LFTs.
* **Blood Pressure:** Monitor for changes, especially in patients on antihypertensives.
* **Hydration Status:** Monitor for signs of dehydration, particularly with dicyclomine's anticholinergic effects.
### Clinical Pearls
* Meftal-Spas should be used for the shortest duration necessary to control symptoms.
* Administer with food or milk to minimize GI upset.
* Educate patients on signs and symptoms of GI bleeding and potential renal/hepatic toxicity.
* Avoid in patients with a history of NSAID-induced asthma or allergic reactions.
* Dicyclomine's anticholinergic effects can be exacerbated by other medications with similar properties.
* Due to potential side effects, especially in the elderly and those with comorbidities, alternative treatments may be preferred.
