# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and hyoscine butylbromide, an antispasmodic agent.
## Primary Indications
Meftal Spas is indicated for the relief of mild to moderate pain associated with smooth muscle spasms, such as menstrual cramps (dysmenorrhea), gastrointestinal spasms, and biliary colic.
## Adult Dosing
The typical adult dose is one tablet orally every 8 hours as needed for pain. Do not exceed 3 tablets in 24 hours.
## Pediatric Dosing
Dosing for pediatric patients is not well-established for this combination product. Mefenamic acid pediatric dosing varies by age and weight. Hyoscine butylbromide pediatric dosing is also weight-based but generally not recommended for this age group due to potential adverse effects. Consultation with a pediatrician is recommended for appropriate treatment of pediatric patients.
## Dose Adjustments
* **Renal Impairment:** Use with caution in patients with renal impairment. Mefenamic acid is renally excreted.
* **Hepatic Impairment:** Use with caution in patients with hepatic impairment. Mefenamic acid is metabolized in the liver.
## Contraindications
* Hypersensitivity to mefenamic acid, hyoscine butylbromide, or any component of the formulation.
* Active gastrointestinal ulceration or bleeding.
* Severe renal or hepatic impairment.
* History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Certain types of glaucoma (narrow-angle).
* Myasthenia gravis.

## Adverse Effects

* **Serious:** Gastrointestinal bleeding, ulceration, perforation; renal toxicity; hepatic toxicity; hypersensitivity reactions (including anaphylaxis); cardiovascular events (NSAID-related); blurred vision, dry mouth, urinary retention (anticholinergic effects).
## Key Drug Interactions
* **NSAIDs:** Increased risk of gastrointestinal bleeding and renal toxicity when co-administered with other NSAIDs or aspirin.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal bleeding.
* **Diuretics and Antihypertensives:** NSAIDs can reduce their efficacy and increase the risk of nephrotoxicity.
* **Lithium:** NSAIDs can increase lithium levels.
* **Methotrexate:** NSAIDs can increase methotrexate toxicity.
* **Anticholinergics:** Additive anticholinergic effects.
* **MAO Inhibitors:** Use with caution.
* **Dopamine antagonists:** May increase risk of extrapyramidal side effects.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding or ulceration.
* Monitor renal and hepatic function, especially in patients with pre-existing impairment or those on long-term therapy.
* Monitor for signs of fluid retention or edema.
* Monitor for cardiovascular risk factors.
## Clinical Pearls
* Meftal Spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to the potential for drowsiness and dizziness, patients should exercise caution when operating machinery or driving.
* Avoid concurrent use with other NSAIDs unless specifically indicated and monitored closely.
* The anticholinergic effects of hyoscine butylbromide can be more pronounced in the elderly.
