# Meftal Spas
## Overview
Meftal Spas is a combination product containing **mefenamic acid** (a nonsteroidal anti-inflammatory drug - NSAID) and **hyoscine butylbromide** (an antispasmodic).
## Primary Indications
* Symptomatic relief of abdominal pain and cramps associated with irritable bowel syndrome (IBS) and other gastrointestinal disorders.
* Pain associated with dysmenorrhea.
## Adult Dosing
* **For GI spasms/pain:** 1 tablet (250 mg mefenamic acid / 10 mg hyoscine butylbromide) three times daily, taken orally. Maximum dose: 4 tablets per day.
* **For dysmenorrhea:** 1 tablet (250 mg mefenamic acid / 10 mg hyoscine butylbromide) three times daily, taken orally, starting from the onset of pain and continuing for 2-3 days. Maximum dose: 4 tablets per day.
## Pediatric Dosing
* Safety and efficacy in children are not established. Use is generally not recommended.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is renally excreted. Significant impairment may require dose reduction or avoidance. Specific guidelines are lacking.
* **Hepatic Impairment:** Use with caution. Mefenamic acid is metabolized by the liver. Dose adjustments may be necessary. Specific guidelines are lacking.
* **Elderly:** Increased risk of adverse effects, particularly GI bleeding, renal, and cardiovascular events. Use with caution and at the lowest effective dose.
## Contraindications
* Hypersensitivity to mefenamic acid, hyoscine butylbromide, or any excipients.
* History of asthma, urticaria, or allergic rhinitis precipitated by NSAIDs or aspirin.
* Severe renal impairment.
* Severe hepatic impairment.
* Severe heart failure.
* Prostatic hypertrophy with urinary retention.
* Tachycardia.
* Gastrointestinal obstruction or paralytic ileus.
* Myasthenia gravis.
* Glaucoma (especially narrow-angle).
* Third trimester of pregnancy.
## Adverse Effects
* **Common (Mefenamic Acid):** Diarrhea, nausea, vomiting, abdominal pain, dyspepsia, constipation.
* **Common (Hyoscine Butylbromide):** Dry mouth, blurred vision, tachycardia, urinary retention, constipation.
* **Serious (Mefenamic Acid):** GI bleeding, ulceration, perforation, cardiovascular thrombotic events, renal failure, hepatic failure, hypersensitivity reactions.
* **Serious (Hyoscine Butylbromide):** Severe anticholinergic effects, especially in overdose or with concurrent anticholinergic medications.
## Key Drug Interactions
* **NSAIDs:** Increased risk of GI toxicity, renal toxicity, and hyperkalemia when used with other NSAIDs, aspirin, or corticosteroids.
* **Anticoagulants (e.g., Warfarin):** Increased risk of bleeding.
* **ACE Inhibitors/ARBs:** May decrease antihypertensive effect and increase risk of renal impairment.
* **Diuretics:** May decrease diuretic and antihypertensive effects and increase risk of renal impairment.
* **Lithium:** Mefenamic acid can increase serum lithium levels.
* **Methotrexate:** Mefenamic acid can decrease methotrexate clearance.
* **Anticholinergics:** Additive effects with hyoscine butylbromide.
* **Dopamine Antagonists:** May reduce the effects of metoclopramide and domperidone.
* **Potassium Supplements:** Increased risk of hyperkalemia.
## Monitoring
* **GI Bleeding:** Monitor for signs and symptoms (e.g., melena, hematemesis).
* **Renal Function:** Especially in patients with pre-existing renal disease, elderly patients, or those on concomitant nephrotoxic drugs.
* **Hepatic Function:** Monitor for signs of liver dysfunction.
* **Blood Pressure:** Monitor for changes, especially in patients with hypertension.
* **Urinary Retention:** Monitor in susceptible individuals.
## Clinical Pearls
* Meftal Spas should be used at the lowest effective dose for the shortest duration necessary to control symptoms.
* Advise patients to avoid concomitant use of other NSAIDs, including aspirin, unless specifically directed by a healthcare professional.
* Due to potential anticholinergic side effects, caution is advised in the elderly and those with pre-existing conditions like glaucoma or prostatic hypertrophy.
* Diarrhea is a common side effect of mefenamic acid and may necessitate dose reduction or discontinuation.
