# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and dicyclomine hydrochloride, an anticholinergic antispasmodic.
## Primary Indications
* Pain and spasms associated with irritable bowel syndrome (IBS).
* Abdominal cramps and pain.
## Adult Dosing
* **Mefenamic Acid:** 500 mg every 8 hours as needed. Maximum daily dose: 1500 mg.
* **Dicyclomine Hydrochloride:** 20 mg every 8 hours as needed. Maximum daily dose: 80 mg.

## Pediatric Dosing

## Dose Adjustments

## Contraindications

## Adverse Effects
* **Common (Mefenamic Acid):** Diarrhea, nausea, vomiting, abdominal pain, dyspepsia, constipation, headache, dizziness.
* **Common (Dicyclomine):** Dry mouth, blurred vision, urinary retention, constipation, drowsiness, confusion, dizziness.
* **Serious:** Gastrointestinal bleeding, ulceration, perforation; renal failure; hepatic failure; cardiovascular thrombotic events; Stevens-Johnson syndrome, toxic epidermal necrolysis.
## Key Drug Interactions
* **NSAIDs:** Increased risk of GI toxicity, renal toxicity.
* **Anticoagulants (e.g., Warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of GI bleeding.
* **Diuretics, ACE inhibitors, ARBs:** Increased risk of renal impairment.
* **Lithium, Methotrexate:** Increased serum levels and toxicity.
* **Other Anticholinergics:** Additive anticholinergic effects.
* **Amantadine, Quinidine, Tricyclic Antidepressants:** Additive anticholinergic effects.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding (e.g., melena, hematemesis).
* Monitor renal and hepatic function, especially with prolonged use or in patients with pre-existing impairment.
* Monitor for anticholinergic adverse effects.
## Clinical Pearls
* Meftal Spas should be taken with food or milk to minimize gastrointestinal upset.
