# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and dicyclomine hydrochloride, an anticholinergic antispasmodic.
## Primary Indications
* Management of abdominal pain associated with visceral spasms, particularly in conditions like irritable bowel syndrome (IBS).
* Symptomatic relief of dysmenorrhea.
## Adult Dosing
* **Abdominal Pain/Spasms:** Typically 1 tablet (mefenamic acid 250 mg, dicyclomine HCl 20 mg) every 8 hours as needed. Do not exceed 4 tablets in 24 hours.
* **Dysmenorrhea:** Typically 1 tablet every 8 hours starting from the onset of pain and bleeding, for up to 3 days.
## Pediatric Dosing
Dosing for Meftal Spas in pediatric patients is not well-established. Mefenamic acid has been used in children, but dicyclomine's use in this population requires extreme caution due to potential anticholinergic side effects. Consult specific pediatric guidelines or product labeling if considering use.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is primarily renally excreted. Dose reduction may be necessary in severe impairment.
* **Hepatic Impairment:** Use with caution. Mefenamic acid can exacerbate hepatic disease.
* **Elderly:** Use with caution, increased risk of GI and renal adverse effects.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or other NSAIDs/anticholinergics.
* History of asthma, urticaria, or allergic-type reactions after aspirin or other NSAIDs.
* Intestinal obstruction, paralytic ileus, glaucoma, myasthenia gravis, severe ulcerative colitis, toxic megacolon, reflux esophagitis, or unstable cardiac disease.
* Undiagnosed abdominal pain.
* Obstructive uropathy.
* Prostatic hypertrophy.
* Third trimester of pregnancy.
## Adverse Effects
* **Mefenamic Acid:** Gastrointestinal distress (nausea, vomiting, diarrhea, abdominal pain, ulceration, bleeding), dizziness, headache, rash, renal dysfunction.
* **Dicyclomine:** Anticholinergic effects (dry mouth, blurred vision, urinary retention, constipation, drowsiness, confusion, tachycardia).
## Key Drug Interactions
* **NSAIDs:** Increased risk of GI bleeding when used concurrently with corticosteroids, SSRIs, or other NSAIDs.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Diuretics and Antihypertensives:** NSAIDs can reduce their efficacy and increase the risk of renal damage.
* **Lithium:** Mefenamic acid can increase lithium levels.
* **Methotrexate:** Mefenamic acid can increase methotrexate toxicity.
## Monitoring
* **Renal function:** Especially in patients with pre-existing renal disease or those on concomitant nephrotoxic agents.
* **Hepatic function:** Especially in patients with liver disease.
* **Gastrointestinal symptoms:** Monitor for signs of bleeding or ulceration.
* **Signs of anticholinergic toxicity:** Particularly in elderly patients or those with underlying conditions.
## Clinical Pearls
* Meftal Spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to anticholinergic effects, caution should be exercised when operating machinery or driving.
* The duration of treatment should be limited to the shortest effective period, especially for dysmenorrhea, due to potential GI risks.
* Concomitant use of dicyclomine with other anticholinergic agents may lead to increased adverse effects.
