# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid (a nonsteroidal anti-inflammatory drug, NSAID) and dicyclomine hydrochloride (an anticholinergic/antispasmodic agent).
## Primary Indications
* Symptomatic relief of mild to moderate pain associated with smooth muscle spasms, such as in dysmenorrhea.
## Adult Dosing
* **Meftal Spas Tablet:** Typically, 1 tablet (containing mefenamic acid 250 mg and dicyclomine HCl 10 mg) every 8 hours as needed. Maximum duration of use is generally limited to 7 days for pain.
* **Meftal Spas Suspension:** Typically, 5 mL to 10 mL every 8 hours as needed.
## Pediatric Dosing
* **Meftal Spas Suspension:** Dosing varies by age and weight. A common pediatric dose for children 6 months to 12 years is 0.5 mL/kg per dose, given every 8 hours. **Always consult specific pediatric guidelines or product labeling for precise dosing based on age and weight.** Maximum daily dose for children should not exceed the adult equivalent.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is renally excreted and can cause renal damage.
* **Hepatic Impairment:** Use with caution.
* **Elderly:** Increased risk of adverse effects, particularly gastrointestinal bleeding and renal events.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or other NSAIDs/anticholinergics.
* Patients with active peptic ulceration or inflammatory bowel disease.
* Patients with intestinal obstruction or paralytic ileus.
* Uncontrolled glaucoma.
* Severe renal or hepatic impairment.

## Adverse Effects
* **Common (Mefenamic Acid):** Gastrointestinal upset (nausea, vomiting, diarrhea, abdominal pain), dyspepsia, headache.
* **Common (Dicyclomine):** Dry mouth, blurred vision, constipation, urinary retention, dizziness, drowsiness.
* **Serious:** Gastrointestinal bleeding, perforation, renal papillary necrosis, exacerbation of asthma, myocardial infarction, stroke, severe skin reactions (e.g., Stevens-Johnson syndrome).
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Other NSAIDs/Aspirin:** Increased risk of GI toxicity.
* **Corticosteroids:** Increased risk of GI ulceration and bleeding.
* **ACE inhibitors, ARBs, Diuretics:** May reduce antihypertensive effect and increase risk of renal impairment.
* **Lithium:** Mefenamic acid can increase lithium levels.
* **Anticholinergics:** Additive effects with dicyclomine, increasing risk of anticholinergic side effects.
* **CNS depressants:** Additive sedative effects.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding (e.g., melena, hematemesis).
* Monitor renal function, especially in patients with pre-existing renal disease or those on concomitant nephrotoxic agents.
* Monitor for signs of fluid retention and edema.
* Assess for signs of anticholinergic toxicity (e.g., confusion, urinary retention).
## Clinical Pearls
* Meftal Spas should be taken with food or milk to minimize gastrointestinal upset.
* Dicyclomine's anticholinergic effects can impair mental alertness; patients should exercise caution when driving or operating machinery.
* Long-term use of mefenamic acid is generally not recommended due to potential for serious adverse effects.
* The combination should be used for the shortest duration necessary to control symptoms.
