# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and hyoscine butylbromide, an antispasmodic agent.
## Primary Indications
* Symptomatic relief of abdominal pain associated with spasmodic conditions of the gastrointestinal tract.
* May be used for menstrual pain.
## Adult Dosing
* **Mefenamic acid:** 500 mg orally every 8 hours as needed.
* **Hyoscine butylbromide:** Typically 10 mg to 20 mg orally up to three times daily.
* Specific dosing for Meftal Spas may vary; consult product labeling or local guidelines. The usual recommended dose for the combination product is one tablet three times daily.
## Pediatric Dosing
* Mefenamic acid:
* 10 to 12 years: 250 mg orally every 8 hours.
* Safety and efficacy in younger children are not well established.
* Hyoscine butylbromide: Dosing in children is not well established.
* Meftal Spas is generally not recommended for children unless specifically indicated by a healthcare provider.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is primarily eliminated renally.
* **Hepatic Impairment:** Use with caution. NSAIDs can exacerbate liver dysfunction.
* Elderly: Increased risk of adverse effects, especially gastrointestinal bleeding and renal toxicity.
## Contraindications
* Known hypersensitivity to mefenamic acid, hyoscine butylbromide, or any component of the formulation.
* History of asthma, urticaria, or allergic-type reactions after aspirin or other NSAIDs.
* Inflammatory bowel disease (e.g., Crohn's disease, ulcerative colitis).
* Obstructive gastrointestinal conditions.
* Myasthenia gravis.
* Tachyarrhythmias.
* Glaucoma (specifically angle-closure glaucoma for hyoscine butylbromide).
* Prostatic hypertrophy with urinary retention.
* Severe renal or hepatic impairment.
## Adverse Effects
* **Mefenamic Acid:** Gastrointestinal upset (nausea, vomiting, diarrhea, constipation), abdominal pain, dyspepsia, headache, dizziness, rash, renal toxicity, gastrointestinal bleeding, exacerbation of asthma.
* **Hyoscine Butylbromide:** Dry mouth, blurred vision, urinary retention, constipation, drowsiness, tachycardia.
* Combination product may exhibit adverse effects of either component.
## Key Drug Interactions
* **Mefenamic Acid:**
* Anticoagulants (e.g., warfarin): Increased risk of bleeding.
* Corticosteroids: Increased risk of gastrointestinal bleeding.
* Other NSAIDs and aspirin: Increased risk of adverse effects.
* SSRIs: Increased risk of gastrointestinal bleeding.
* Lithium, Methotrexate: Increased serum levels and toxicity.
* **Hyoscine Butylbromide:**
* Anticholinergics: Potentiated effects.
* Dopamine antagonists (e.g., metoclopramide): May reduce efficacy of both.
* Tricyclic antidepressants, MAOIs, antihistamines: May potentiate anticholinergic effects.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding.
* Assess renal and hepatic function periodically, especially with prolonged use or in at-risk patients.
* Monitor for signs of hypersensitivity reactions.
* Assess for effectiveness in pain relief.
## Clinical Pearls
* Meftal Spas is a combination product, and its use should be guided by the indications for both active ingredients.
* Administer with food or milk to minimize gastrointestinal upset from mefenamic acid.
* Avoid concurrent use with other NSAIDs.
* Caution in patients with pre-existing cardiovascular disease, hypertension, or risk factors for cardiovascular events due to mefenamic acid.
* Hyoscine butylbromide's anticholinergic effects can be problematic in certain patient populations (e.g., elderly, those with glaucoma or urinary retention).
