# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and dicyclomine hydrochloride, an anticholinergic antispasmodic.
## Primary Indications
* Symptomatic relief of mild to moderate pain associated with smooth muscle spasms, particularly in conditions like dysmenorrhea and gastrointestinal disorders.
## Adult Dosing
* **Mefenamic Acid:** 500 mg orally every 8 hours as needed for pain.
* **Dicyclomine Hydrochloride:** 20 mg orally every 8 hours as needed for spasms.
* The combination product typically provides these doses per tablet or capsule.
* Maximum duration of use for mefenamic acid is generally 7 days.
* Maximum dose for dicyclomine is typically 80 mg per day.
## Pediatric Dosing
* Dosing for Meftal Spas in pediatric populations is not well-established. Mefenamic acid is generally not recommended for children under 14 years of age for more than short-term use. Dicyclomine should be used cautiously in pediatric patients due to potential for anticholinergic side effects. Consult specific pediatric guidelines or a pediatrician for appropriate management.
## Dose Adjustments
* **Renal Impairment:** Mefenamic acid should be used with caution and potentially at reduced doses in patients with significant renal impairment due to the risk of NSAID-induced nephrotoxicity.
* **Hepatic Impairment:** Mefenamic acid should be used with caution in patients with hepatic impairment.
* **Elderly:** Elderly patients may be more susceptible to the adverse effects of NSAIDs, including gastrointestinal bleeding and renal effects.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or other NSAIDs.
* History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Inflammatory bowel disease.
* Obstructive uropathy or gastrointestinal obstruction.
* Myasthenia gravis.
* Severe renal or hepatic impairment.
* Active gastrointestinal bleeding or perforation.
* Pregnancy (especially third trimester).
* Breastfeeding.
## Adverse Effects
* **Mefenamic Acid:** Gastrointestinal upset (nausea, vomiting, diarrhea, abdominal pain), dyspepsia, heartburn, dizziness, headache, drowsiness, rash, potential for renal toxicity, increased risk of cardiovascular thrombotic events, and serious gastrointestinal bleeding or perforation.
* **Dicyclomine Hydrochloride:** Dry mouth, blurred vision, urinary retention, constipation, drowsiness, dizziness, confusion, and in severe cases, hallucinations or exacerbation of glaucoma.
## Key Drug Interactions
* **NSAIDs:** Increased risk of gastrointestinal bleeding when used with other NSAIDs, corticosteroids, anticoagulants (e.g., warfarin), or selective serotonin reuptake inhibitors (SSRIs).
* **Antihypertensives:** NSAIDs can reduce the effectiveness of antihypertensive medications.
* **Diuretics:** Increased risk of renal impairment.
* **Lithium:** NSAIDs can increase serum lithium levels.
* **Methotrexate:** NSAIDs can increase methotrexate toxicity.
* **Anticholinergics:** Additive anticholinergic effects when used with other agents possessing anticholinergic properties (e.g., tricyclic antidepressants, antihistamines).
## Monitoring
* Monitor for signs of gastrointestinal bleeding (e.g., melena, hematemesis).
* Monitor renal function, especially in patients with pre-existing renal disease or those on concomitant nephrotoxic agents.
* Monitor for signs of hypersensitivity reactions.
* Monitor for efficacy and adverse effects of dicyclomine (e.g., dry mouth, blurred vision, urinary retention).
## Clinical Pearls
* Meftal Spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to the anticholinergic effects of dicyclomine, patients should be advised about potential risks such as impaired judgment, blurred vision, and difficulty urinating, especially when operating machinery or driving.
* The duration of treatment should be as short as possible and guided by patient response.
* Avoid concurrent use of other NSAIDs or salicylates.
