# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid (a nonsteroidal anti-inflammatory drug, NSAID) and dicyclomine hydrochloride (an anticholinergic antispasmodic).
## Primary Indications
* Symptomatic relief of mild to moderate pain associated with smooth muscle spasms, such as in dysmenorrhea and gastrointestinal colic.
## Adult Dosing
* **Mefenamic Acid:** 500 mg every 8 hours as needed. Maximum daily dose is 1500 mg for short-term use.
* **Dicyclomine Hydrochloride:** 20 mg every 8 hours as needed.
Dosing and frequency may vary based on local protocol and patient response.
## Pediatric Dosing
* Mefenamic acid is not generally recommended for pediatric use due to limited data and potential for adverse effects.
* Dicyclomine hydrochloride pediatric dosing varies by age and weight. ## Dose Adjustments

## Contraindications

* Inflammatory bowel disease.
* Gastrointestinal obstruction, paralytic ileus, myasthenia gravis, severe renal or hepatic impairment, glaucoma, tachycardia, obstructive uropathy.
* Use in the third trimester of pregnancy.
## Adverse Effects
* **Common (Mefenamic Acid):** Diarrhea, nausea, vomiting, abdominal pain, constipation, dyspepsia.
* **Common (Dicyclomine):** Dry mouth, blurred vision, urinary retention, constipation, drowsiness, dizziness.
* **Serious:** Gastrointestinal bleeding, ulceration, perforation; renal papillary necrosis, interstitial nephritis, glomerulonephritis; hepatic failure; cardiovascular thrombotic events; hypersensitivity reactions; exacerbation of asthma.
## Key Drug Interactions
* **NSAIDs:** Increased risk of bleeding with anticoagulants (e.g., warfarin) and antiplatelet agents (e.g., aspirin, clopidogrel). Increased risk of renal toxicity with ACE inhibitors, ARBs, diuretics, and cyclosporine.
* **Dicyclomine:** Additive anticholinergic effects with other anticholinergic agents (e.g., tricyclic antidepressants, antihistamines, antiparkinsonian drugs). May decrease the absorption of drugs that rely on gastric emptying for absorption (e.g., digoxin).
## Monitoring
* **GI Bleeding:** Monitor for signs and symptoms (e.g., melena, hematemesis).
* **Renal Function:** Assess baseline and periodically monitor renal function, especially in patients with pre-existing renal disease or those at risk.
* **Hepatic Function:** Monitor liver enzymes if liver disease is present or if signs of hepatic dysfunction occur.
* **Signs of Anticholinergic Toxicity:** Monitor for dry mouth, blurred vision, urinary retention, and cognitive impairment.
## Clinical Pearls
* Meftal Spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to the anticholinergic effects, caution should be exercised when driving or operating machinery.
* Prolonged use of mefenamic acid is not recommended due to the risk of serious adverse effects.
* Discontinue if diarrhea is persistent or severe.
