## Overview
Meftal Spas is a brand name for a combination medication containing mefenamic acid (a nonsteroidal anti-inflammatory drug - NSAID) and hyoscine butylbromide (an anticholinergic antispasmodic). Mefenamic acid reduces inflammation and pain, while hyoscine butylbromide relaxes smooth muscles in the gastrointestinal tract.
## Primary Indications
* Abdominal pain associated with spasms
* Menstrual cramps (dysmenorrhea)
## Adult Dosing
* **Mefenamic Acid:** Typically 250 mg to 500 mg every 8 hours. Maximum daily dose is 1500 mg.
* **Hyoscine Butylbromide:** Typically 10 mg to 20 mg every 8 hours.

## Pediatric Dosing

* **Common (Hyoscine Butylbromide):** Dry mouth, blurred vision, constipation, urinary retention, tachycardia, sweating decreased.
* **Serious (Hyoscine Butylbromide):** Angle-closure glaucoma, severe allergic reactions.
## Key Drug Interactions
* **NSAIDs (e.g., aspirin, ibuprofen):** Increased risk of GI toxicity.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Diuretics and antihypertensives:** May reduce efficacy and increase risk of renal toxicity.
* **Corticosteroids:** Increased risk of GI adverse effects.
* **SSRIs/SNRIs:** Increased risk of GI bleeding.
* **Other anticholinergics:** Additive anticholinergic effects.
* **Opioids:** May mask symptoms of GI obstruction or toxicity.
* **Metoclopramide:** May antagonize hyoscine butylbromide's effects.
## Monitoring
* **Renal function:** Monitor creatinine and BUN, especially in patients with pre-existing renal disease or those on long-term therapy.
* **Hepatic function:** Monitor LFTs if signs of hepatic dysfunction appear.
* **Gastrointestinal symptoms:** Assess for signs of bleeding (e.g., melena, hematemesis) or ulceration.
* **Blood pressure:** Especially in patients with hypertension.
* **Fluid balance:** Monitor for signs of fluid retention.
## Clinical Pearls
* Use the lowest effective dose for the shortest possible duration to minimize risks.
* Administer with food or milk to reduce GI upset.
* Mefenamic acid should not be used for more than 7 days consecutively due to potential for serious adverse effects.
* Be aware of the anticholinergic side effects of hyoscine butylbromide, especially in the elderly.
* Discontinue immediately if signs of GI bleeding, renal impairment, or severe allergic reaction occur.
