# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid (a non-steroidal anti-inflammatory drug - NSAID) and dicyclomine hydrochloride (an anticholinergic/antispasmodic). Mefenamic acid provides analgesic and anti-inflammatory effects, while dicyclomine reduces smooth muscle spasms.
## Primary Indications
* Painful menstruation (dysmenorrhea)
* Abdominal pain associated with smooth muscle spasms
## Adult Dosing
* **Meftal Spas tablets:** 1 tablet (250 mg mefenamic acid / 20 mg dicyclomine) every 8 hours as needed.
* Maximum mefenamic acid dose: 1500 mg/day.
* Maximum dicyclomine dose: 40 mg/day.
## Pediatric Dosing
Dosing for Meftal Spas in pediatric patients is not well-established. Use with caution and consider alternative agents if available and appropriate.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is renally excreted; dose reduction may be necessary in severe impairment.
* **Hepatic Impairment:** Use with caution. NSAIDs can exacerbate hepatic dysfunction.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or other NSAIDs.
* History of asthma, urticaria, or allergic-type reactions after aspirin or other NSAIDs.
* Intestinal obstruction, paralytic ileus, severe ulcerative colitis, toxic megacolon.
* Glaucoma.
* Myasthenia gravis.
* Severe renal or hepatic impairment.
* Active peptic ulceration or inflammatory bowel disease.
* Concurrent use with other NSAIDs or aspirin.
## Adverse Effects
* **Gastrointestinal:** Nausea, vomiting, diarrhea, abdominal pain, dyspepsia, constipation, GI bleeding, ulceration, perforation.
* **Central Nervous System:** Dizziness, headache, drowsiness, confusion.
* **Anticholinergic effects (dicyclomine):** Dry mouth, blurred vision, urinary retention, constipation, decreased sweating.
* **Cardiovascular:** Edema, hypertension, potential increased risk of cardiovascular thrombotic events with NSAIDs.
* **Renal:** Renal impairment, interstitial nephritis.
* **Hematologic:** Anemia, leukopenia, thrombocytopenia.
## Key Drug Interactions
* **Other NSAIDs/Aspirin:** Increased risk of GI bleeding and other adverse effects.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of GI ulceration and bleeding.
* **Diuretics and Antihypertensives:** May reduce efficacy and increase risk of renal impairment.
* **Lithium:** NSAIDs can increase serum lithium levels.
* **SSRIs/SNRIs:** Increased risk of GI bleeding.
* **Anticholinergics:** Additive anticholinergic effects.
## Monitoring
* Monitor for signs and symptoms of GI bleeding (e.g., melena, hematemesis).
* Assess renal and hepatic function, especially in patients with pre-existing impairment or those on long-term therapy.
* Monitor blood pressure.
* Observe for signs of anticholinergic toxicity.
## Clinical Pearls
* Meftal Spas should be taken with food or milk to minimize GI upset.
* Avoid concurrent use with other NSAIDs.
* Dicyclomine can cause drowsiness; caution patients about driving or operating machinery.
* Discontinue if severe adverse effects occur.
* The risk of cardiovascular thrombotic events associated with NSAIDs should be considered.
