# Meftal Spas
## Overview
Meftal Spas is a combination medication containing **mefenamic acid**, a nonsteroidal anti-inflammatory drug (NSAID), and **dicyclomine hydrochloride**, an anticholinergic antispasmodic agent.
## Primary Indications
* Symptomatic relief of mild to moderate pain associated with smooth muscle spasm, such as in irritable bowel syndrome, diverticulitis, and biliary or renal colic.
* Management of dysmenorrhea.
## Adult Dosing
* **Pain/Spasm:** Typically 1 capsule (mefenamic acid 250 mg, dicyclomine HCl 10 mg) every 8 hours as needed.
* **Dysmenorrhea:** Typically 1 capsule every 8 hours, starting at the onset of pain and continuing for the duration of the symptoms.
* Maximum daily dose: Not explicitly defined for the combination, but individual components have limits. For mefenamic acid, typically 750 mg in the first 24 hours, then 500 mg every 6-8 hours thereafter. For dicyclomine, typically 40 mg per day.
## Pediatric Dosing
* Dosing for Meftal Spas in pediatric patients has not been established. Use in children is generally not recommended due to the potential for serious adverse effects.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is renally excreted.
* **Hepatic Impairment:** Use with caution. Mefenamic acid can affect liver function.
* **Elderly:** Increased risk of gastrointestinal bleeding, renal, cardiovascular, and hepatic adverse effects. Consider lower doses and closer monitoring.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or other NSAIDs.
* Patients with a history of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Inflammatory bowel disease (e.g., ulcerative colitis, Crohn's disease).
* Gastrointestinal obstruction, paralytic ileus, severe ulcerative colitis, toxic megacolon.
* Myasthenia gravis.
* Glaucoma (narrow-angle).
* Severe renal or hepatic impairment.
* Known risk factors for cardiovascular thrombotic events.
* Third trimester of pregnancy.
## Adverse Effects
* **Common:** Nausea, vomiting, abdominal pain, diarrhea, constipation, dizziness, headache, dry mouth, blurred vision, urinary retention, drowsiness.
* **Serious (NSAID-related):** Gastrointestinal bleeding, ulceration, perforation; cardiovascular thrombotic events (MI, stroke); renal failure; hepatic toxicity; hypersensitivity reactions.
* **Serious (Anticholinergic-related):** Delirium, confusion, hallucinations (especially in elderly), severe dry mouth, constipation, urinary retention.
## Key Drug Interactions
* **NSAIDs:** Increased risk of gastrointestinal bleeding when used with corticosteroids, anticoagulants (e.g., warfarin), SSRIs, or other NSAIDs.
* **ACE inhibitors, ARBs, diuretics:** Reduced antihypertensive effect and increased risk of renal impairment.
* **Lithium:** Increased serum lithium levels.
* **Methotrexate:** Increased risk of methotrexate toxicity.
* **Anticholinergics:** Additive anticholinergic effects with other drugs having anticholinergic properties (e.g., tricyclic antidepressants, antihistamines, antipsychotics).
* **Antacids:** May decrease absorption of dicyclomine.
## Monitoring
* **Renal function:** Baseline and periodic assessment, especially in patients with pre-existing renal disease or those at risk.
* **Liver function:** Baseline and periodic assessment, particularly with long-term use or in patients with hepatic impairment.
* **Gastrointestinal symptoms:** Monitor for signs and symptoms of bleeding (e.g., melena, hematemesis) or ulceration.
* **Cardiovascular risk factors:** Monitor for signs of cardiovascular events.
* **Anticholinergic effects:** Monitor for dry mouth, blurred vision, urinary retention, constipation, and cognitive changes, especially in the elderly.
## Clinical Pearls
* Meftal Spas should be used at the lowest effective dose for the shortest duration necessary to control symptoms.
* Administer with food or milk to minimize gastrointestinal upset.
* Caution is advised in patients with a history of asthma, as NSAIDs can trigger bronchospasm.
* Anticholinergic effects are more pronounced in the elderly.
* Discontinue use if significant GI bleeding or ulceration occurs.
