# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and dicyclomine hydrochloride, an anticholinergic antispasmodic.
## Primary Indications
* Symptomatic relief of spasms associated with functional gastrointestinal disorders.
* Painful menstruation (dysmenorrhea).
## Adult Dosing
* **Functional GI Disorders:** 1 tablet (250 mg mefenamic acid / 20 mg dicyclomine HCl) every 8 hours. Maximum 3 tablets per day.
* **Dysmenorrhea:** 1 tablet every 8 hours starting from the onset of pain and bleeding. Typically used for 2-3 days. Maximum 3 tablets per day.
## Pediatric Dosing
Dosing is not well-established for pediatric patients. Use in children should be carefully considered and based on the risk-benefit profile, with doses guided by specialist recommendations.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is primarily excreted by the kidneys. Reduced doses may be necessary.
* **Hepatic Impairment:** Use with caution. Mefenamic acid is metabolized by the liver. Reduced doses may be necessary.
* **Elderly:** Elderly patients may be more susceptible to adverse effects, particularly gastrointestinal bleeding and renal toxicity. Use with caution and consider lower doses.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or any component of the formulation.
* Patients with a history of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Inflammatory bowel disease (e.g., ulcerative colitis, Crohn's disease).
* Gastrointestinal obstruction or paralytic ileus.
* Uncontrolled glaucoma.
* Myasthenia gravis.
* Severe renal or hepatic impairment.
* Severe heart failure.
* During the perioperative period of coronary artery bypass graft (CABG) surgery.
## Adverse Effects
Common adverse effects include:
* **GI:** Nausea, vomiting, diarrhea, abdominal pain, dyspepsia, constipation, gastrointestinal bleeding (potentially severe and life-threatening).
* **CNS:** Dizziness, drowsiness, headache.
* **Anticholinergic:** Dry mouth, blurred vision, urinary retention, constipation, confusion (especially in the elderly).
* **Other:** Rash, tinnitus, renal impairment, increased blood pressure.
## Key Drug Interactions
* **Other NSAIDs (including aspirin):** Increased risk of gastrointestinal toxicity.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal bleeding.
* **SSRIs/SNRIs:** Increased risk of gastrointestinal bleeding.
* **Diuretics and antihypertensives:** NSAIDs can reduce the efficacy of these drugs and increase the risk of renal impairment.
* **Lithium:** NSAIDs can increase lithium levels.
* **Methotrexate:** NSAIDs can increase methotrexate toxicity.
* **Anticholinergic agents:** Additive anticholinergic effects with dicyclomine.
* **Amantadine, quinidine, tricyclic antidepressants:** Increased anticholinergic effects.
## Monitoring
* **GI bleeding:** Monitor for signs and symptoms of bleeding (e.g., melena, hematemesis, abdominal pain).
* **Renal function:** Monitor serum creatinine and BUN, especially in patients with pre-existing renal disease or those at risk.
* **Hepatic function:** Monitor liver enzymes, especially with prolonged use.
* **Blood pressure:** Monitor blood pressure, especially in patients with hypertension.
* **Fluid and electrolyte balance:** Monitor for signs of fluid retention or dehydration.
## Clinical Pearls
* Meftal Spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to anticholinergic effects, patients should be advised to avoid activities requiring mental alertness (e.g., driving) until they know how the medication affects them.
* The risk of serious cardiovascular thrombotic events, myocardial infarction, and stroke associated with NSAID use may increase with duration of use and in patients with risk factors.
* Use the lowest effective dose for the shortest duration necessary.
