# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid and dicyclomine hydrochloride. Mefenamic acid is a nonsteroidal anti-inflammatory drug (NSAID) used for mild to moderate pain. Dicyclomine is an anticholinergic and antispasmodic agent used to relieve smooth muscle spasms.
## Primary Indications
* Short-term treatment of abdominal pain associated with intestinal colic.
* Symptomatic relief of mild to moderate pain, particularly when associated with smooth muscle spasm.
## Adult Dosing
* **Mefenamic Acid:** 500 mg every 8 hours as needed for pain.
* **Dicyclomine Hydrochloride:** 20 mg every 8 hours as needed for spasms.
The typical dose of Meftal Spas provides both components simultaneously. Maximum dose for mefenamic acid is typically 1500 mg/day and for dicyclomine is typically 80 mg/day. Do not exceed recommended doses.
## Pediatric Dosing
There is no established pediatric dosing for Meftal Spas. Dosing of individual components (mefenamic acid and dicyclomine) in pediatric populations should be determined by a healthcare provider based on age, weight, and clinical condition.
## Dose Adjustments
* **Renal Impairment:** Mefenamic acid should be used with caution in patients with mild to moderate renal impairment. Avoid in severe renal impairment.
* **Hepatic Impairment:** Mefenamic acid should be used with caution in patients with hepatic impairment. Avoid in severe hepatic impairment.
* **Elderly:** Elderly patients may be more susceptible to adverse effects, particularly gastrointestinal bleeding, renal toxicity, and cardiovascular events associated with NSAIDs.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or any component of the formulation.
* Active or history of recurrent peptic ulceration or gastrointestinal bleeding.
* Severe renal or hepatic impairment.
* Conditions where anticholinergic agents are contraindicated, such as:
* Glaucoma (especially narrow-angle glaucoma)
* Myasthenia gravis
* Obstructive uropathy (e.g., bladder neck obstruction)
* Obstructive gastrointestinal disease (e.g., pyloric stenosis, achalasia)
* Unstable cardiovascular status in acute hemorrhage.
* Concurrent use with other NSAIDs, including aspirin.
## Adverse Effects
* **Gastrointestinal:** Nausea, vomiting, diarrhea, abdominal pain, dyspepsia, constipation, GI bleeding, perforation.
* **Central Nervous System:** Dizziness, drowsiness, headache, confusion.
* **Anticholinergic Effects (due to dicyclomine):** Dry mouth, blurred vision, urinary retention, constipation, reduced sweating, drowsiness, dizziness.
* **Cardiovascular:** Edema, hypertension, myocardial infarction, stroke (associated with NSAIDs, especially with prolonged use or in patients with risk factors).
* **Renal:** Renal impairment, interstitial nephritis.
* **Hepatic:** Elevated liver enzymes, hepatitis.
* **Dermatologic:** Rash, pruritus.
## Key Drug Interactions
* **Other NSAIDs (including aspirin):** Increased risk of GI toxicity.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of GI bleeding.
* **SSRIs/SNRIs:** Increased risk of GI bleeding.
* **Diuretics and Antihypertensives:** NSAIDs can reduce their efficacy and increase the risk of renal toxicity.
* **Lithium:** NSAIDs can increase lithium levels.
* **Methotrexate:** NSAIDs can increase methotrexate toxicity.
* **Anticholinergics:** Additive anticholinergic effects if dicyclomine is combined with other drugs having these properties.
* **Amantadine, quinidine, tricyclic antidepressants:** May potentiate anticholinergic effects of dicyclomine.
* **Antacids:** May decrease absorption of dicyclomine.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding, such as melena, hematemesis, or abdominal pain.
* Monitor renal function, especially in patients with pre-existing renal disease, dehydration, or those taking diuretics.
* Monitor liver function tests in patients with hepatic impairment or those on long-term therapy.
* Monitor for signs of anticholinergic effects, particularly in the elderly or those with pre-existing conditions.
## Clinical Pearls
* Use the lowest effective dose for the shortest duration necessary to minimize risks.
* Administer with food or milk to reduce potential gastrointestinal upset.
* Avoid use in patients with a history of NSAID-induced asthma, urticaria, or allergic-type reactions.
* Dicyclomine's anticholinergic effects can be pronounced, especially in the elderly. Use with caution.
* Effectiveness for chronic conditions is not established.
