# Meftal spas
## Overview
Meftal spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and hyoscine butylbromide, an antispasmodic agent.
## Primary Indications
* Symptomatic relief of smooth muscle spasms associated with the gastrointestinal tract, biliary tract, and urinary tract.
* Management of pain and inflammation in conditions such as dysmenorrhea.
## Adult Dosing
* **Mefenamic acid component:** Typically 250 mg to 500 mg every 8 hours as needed. Do not exceed 500 mg per dose or 1500 mg per day.
* **Hyoscine butylbromide component:** Typically 10 mg to 20 mg three times daily.
The specific dosing regimen for Meftal spas will depend on the prescribed strength of each component. Please refer to the product's specific labeling or local prescribing guidelines.
## Pediatric Dosing
Dosing for pediatric patients is not well-established for this combination product. Mefenamic acid is generally not recommended for use in children under 14 years of age without specific guidance from a pediatrician. Hyoscine butylbromide pediatric dosing is also not routinely established and should be guided by a healthcare professional.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is renally excreted and accumulation may occur.
* **Hepatic Impairment:** Use with caution. Mefenamic acid is metabolized by the liver.
* **Elderly:** Use with caution, particularly if there are co-existing conditions such as renal or hepatic impairment. Increased risk of gastrointestinal bleeding and renal adverse effects.
## Contraindications
* Hypersensitivity to mefenamic acid, hyoscine butylbromide, or any component of the formulation.
* Active gastrointestinal ulceration or inflammation.
* History of recurrent gastrointestinal bleeding.
* Severe renal or hepatic impairment.
* Myasthenia gravis.
* Tachycardia.
* Glaucoma (angle-closure).
* Prostatic hypertrophy with urinary retention.
* Ileus or paralytic ileus.
* Co-administration with other NSAIDs, including cyclooxygenase-2 (COX-2) selective inhibitors.
## Adverse Effects
* **Common (Mefenamic acid):** Nausea, vomiting, diarrhea, abdominal pain, dyspepsia, constipation, headache, dizziness.
* **Serious (Mefenamic acid):** Gastrointestinal bleeding, ulceration, perforation, renal impairment, hepatic impairment, cardiovascular thrombotic events, Stevens-Johnson syndrome, toxic epidermal necrolysis.
* **Common (Hyoscine butylbromide):** Dry mouth, blurred vision, constipation, urinary retention, tachycardia.
* **Serious (Hyoscine butylbromide):** Angle-closure glaucoma, hypersensitivity reactions.
## Key Drug Interactions
* **Mefenamic Acid:**
* Anticoagulants (e.g., warfarin): Increased risk of bleeding.
* Corticosteroids: Increased risk of gastrointestinal ulceration and bleeding.
* SSRIs: Increased risk of gastrointestinal bleeding.
* Diuretics, ACE inhibitors, ARBs: Reduced antihypertensive effect and increased risk of renal impairment.
* Lithium: Increased lithium levels.
* Methotrexate: Increased methotrexate toxicity.
* **Hyoscine Butylbromide:**
* Other anticholinergic agents: Potentiated anticholinergic effects.
* Amantadine: Potentiated anticholinergic effects.
* Tricyclic antidepressants: Potentiated anticholinergic effects.
* Dopamine antagonists (e.g., metoclopramide): Reduced efficacy of both.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding (e.g., melena, hematemesis).
* Assess renal and hepatic function periodically, especially with long-term use.
* Monitor blood pressure.
* Evaluate for signs of cardiovascular events.
* Assess for effectiveness in pain and spasm relief.
## Clinical Pearls
* Meftal spas should be used at the lowest effective dose for the shortest duration necessary to control symptoms.
* Risk of gastrointestinal toxicity is increased with higher doses and longer duration of therapy.
* Consider alternative agents if gastrointestinal intolerance or bleeding occurs.
* Advise patients to avoid concurrent use of alcohol, which can increase the risk of gastrointestinal side effects.
