# Meftal spas
## Overview
Meftal spas is a combination medication containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and dicyclomine, an anticholinergic antispasmodic. Mefenamic acid reduces inflammation and pain, while dicyclomine relaxes smooth muscles in the gastrointestinal tract.
## Primary Indications
* Abdominal pain associated with visceral spasms.
* Painful menstruation (dysmenorrhea).
## Adult Dosing
* **Meftal spas:** One tablet orally every 8 hours as needed. Do not exceed 3 tablets per day.
## Pediatric Dosing
* Dosing for pediatric patients is not well-established and should be determined by a healthcare professional based on age, weight, and clinical condition. Caution is advised due to potential adverse effects.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is primarily renally excreted.
* **Hepatic Impairment:** Use with caution.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or other NSAIDs.
* History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Intestinal obstruction, paralytic ileus, ulcerative colitis, toxic megacolon.
* Uncontrolled glaucoma.
* Myasthenia gravis.

* Third trimester of pregnancy.
## Adverse Effects
* **Gastrointestinal:** Nausea, vomiting, diarrhea, abdominal pain, constipation, dyspepsia.
* **Central Nervous System:** Dizziness, headache, drowsiness, blurred vision, confusion.
* **Cardiovascular:** Edema, hypertension (mefenamic acid component).
* **Other:** Rash, dry mouth, urinary retention.
* **Serious:** Gastrointestinal bleeding, perforation, myocardial infarction, stroke, renal failure, severe allergic reactions.
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal adverse effects.
* **Other NSAIDs and Aspirin:** Increased risk of gastrointestinal bleeding and other adverse effects.
* **ACE inhibitors, ARBs, diuretics:** Reduced antihypertensive effect and increased risk of renal impairment.
* **Lithium:** Increased lithium levels.
* **Methotrexate:** Increased methotrexate toxicity.
* **Anticholinergics:** Additive anticholinergic effects.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding (e.g., black, tarry stools, hematemesis).
* Assess for signs of renal impairment (e.g., changes in urine output, elevated creatinine).
* Evaluate for anticholinergic side effects (e.g., dry mouth, constipation, urinary retention, blurred vision).
* Monitor blood pressure.
## Clinical Pearls
* Meftal spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to anticholinergic effects, caution is advised in elderly patients and those with conditions that may be aggravated by anticholinergics (e.g., prostatic hypertrophy).
* Avoid concomitant use with other NSAIDs.
* Discontinue if significant adverse effects occur.
