# Meftal Spas
## Overview
Meftal Spas is a combination medication containing Mefenamic acid and Dicyclomine hydrochloride. Mefenamic acid is a nonsteroidal anti-inflammatory drug (NSAID) used for pain relief. Dicyclomine is an anticholinergic and antispasmodic agent used to relieve smooth muscle spasms.
## Primary Indications
* Abdominal pain associated with smooth muscle spasms, particularly in conditions like irritable bowel syndrome and dysmenorrhea.
## Adult Dosing
* **Mefenamic acid:** 500 mg every 8 hours as needed for pain.
* **Dicyclomine hydrochloride:** 20 mg four times daily, taken 30-60 minutes before meals.
## Pediatric Dosing
Dosing for Meftal Spas is not well-established in pediatric populations. Mefenamic acid is generally not recommended for children under 12 years of age without specific medical guidance. Dicyclomine use in children is also limited and requires careful consideration due to potential anticholinergic side effects.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is renally excreted, and dose adjustments may be necessary, especially in severe impairment.
* **Hepatic Impairment:** Use with caution. Mefenamic acid is metabolized by the liver.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or any component of the formulation.
* Patients with a history of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Inflammatory bowel disease.
* Gastrointestinal obstruction or bleeding.
* Glaucoma.
* Myasthenia gravis.

## Adverse Effects
* **Mefenamic acid:** Gastrointestinal upset, nausea, vomiting, diarrhea, constipation, abdominal pain, dyspepsia, ulceration, bleeding, renal toxicity, hepatic toxicity, rash, dizziness, headache.
* **Dicyclomine:** Dry mouth, blurred vision, constipation, urinary retention, drowsiness, confusion, nausea, dizziness.
## Key Drug Interactions
* **NSAIDs:** Increased risk of gastrointestinal bleeding when used concurrently with other NSAIDs or corticosteroids.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **ACE inhibitors, ARBs, diuretics:** NSAIDs can reduce the antihypertensive effect and increase the risk of renal impairment.
* **Lithium:** NSAIDs can increase lithium levels.
* **Methotrexate:** NSAIDs can increase methotrexate toxicity.
* **Anticholinergics (other):** Additive anticholinergic effects with dicyclomine.
* **Antacids:** May decrease absorption of dicyclomine.
## Monitoring
* Monitor for signs of gastrointestinal bleeding or perforation.
* Assess renal and hepatic function, especially with prolonged use or in patients with pre-existing impairment.
* Monitor for adverse anticholinergic effects (dry mouth, blurred vision, urinary retention, constipation).
## Clinical Pearls
* Meftal Spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to anticholinergic effects, patients should exercise caution when driving or operating machinery.
* Avoid concurrent use with other NSAIDs.
* The efficacy and safety of long-term use have not been established.
