# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and dicyclomine hydrochloride, an anticholinergic antispasmodic.
## Primary Indications
* Symptomatic relief of spasms associated with gastrointestinal conditions.
* Pain and inflammation associated with dysmenorrhea.
## Adult Dosing
* **Gastrointestinal Spasms:** 1 tablet (mefenamic acid 250 mg, dicyclomine HCl 20 mg) every 8 hours as needed. Maximum duration of use is typically 7 days.
* **Dysmenorrhea:** 1 tablet (mefenamic acid 250 mg, dicyclomine HCl 20 mg) every 8 hours starting with the onset of pain and bleeding, for up to 3 days.
## Pediatric Dosing
Dosing for pediatric patients is not well-established and should be determined by a healthcare provider based on clinical judgment and local protocols. Mefenamic acid is generally not recommended for use in children under 14 years of age due to potential risks.
## Dose Adjustments
* **Renal Impairment:** Mefenamic acid is renally excreted and should be used with caution in patients with impaired renal function. Dose reduction may be necessary.
* **Hepatic Impairment:** Mefenamic acid can cause liver dysfunction. Use with caution; dose adjustment is not well-defined but should be guided by clinical response and liver function tests.
* **Elderly:** Elderly patients may be more susceptible to the adverse effects of NSAIDs, including gastrointestinal bleeding, renal, and cardiovascular events. Use with caution.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or any component of the formulation.
* History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Inflammatory bowel disease.
* Gastrointestinal obstruction.
* Severe ulcerative colitis.
* Glaucoma.
* Myasthenia gravis.

* Renal impairment or failure

* Exacerbation of asthma
* CNS effects (confusion, hallucinations)
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal bleeding.
* **Other NSAIDs/Aspirin:** Increased risk of gastrointestinal toxicity and other adverse effects.
* **ACE inhibitors, ARBs, Diuretics:** Reduced antihypertensive efficacy and increased risk of renal impairment.
* **Lithium:** Increased serum lithium levels.
* **Methotrexate:** Increased risk of methotrexate toxicity.
* **Anticholinergics (when combined with dicyclomine):** Additive anticholinergic effects.
* **CNS Depressants:** Additive CNS depression.
## Monitoring
* **Renal function:** Baseline and periodic monitoring, especially in patients with pre-existing renal disease or those on concomitant nephrotoxic agents.
* **Liver function:** Baseline and periodic monitoring, especially in patients with pre-existing liver disease.
* **Gastrointestinal symptoms:** Monitor for signs of bleeding or ulceration.
* **Blood pressure:** Monitor for potential changes, especially with concomitant antihypertensive therapy.
* **Signs of anticholinergic toxicity:** Monitor for dry mouth, blurred vision, urinary retention, constipation, confusion.
## Clinical Pearls
* Meftal Spas combines an anti-inflammatory agent with an antispasmodic. The efficacy of the combination is patient-specific.
* Due to the risk of gastrointestinal bleeding and ulceration associated with NSAIDs, Meftal Spas should be used at the lowest effective dose for the shortest duration necessary.
* Caution is advised in patients with a history of asthma, as NSAIDs can precipitate bronchospasm.
* Anticholinergic side effects are more pronounced in the elderly.
* Avoid concurrent use with other NSAIDs or aspirin unless specifically indicated and closely monitored.
