# Meftal spas
## Overview
Meftal spas is a combination medication containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and hyoscine butylbromide, an antispasmodic agent.
## Primary Indications
* Symptomatic relief of abdominal pain and cramps associated with various gastrointestinal conditions, including irritable bowel syndrome (IBS) and other functional bowel disorders.
* Management of dysmenorrhea.
## Adult Dosing
* **Usual dose:** 1 tablet (mefenamic acid 250 mg / hyoscine butylbromide 10 mg) three times daily.
* **Maximum dose:** Not explicitly defined for the combination product, but individual component maximums should be considered. For mefenamic acid, the maximum daily dose is typically 1000 mg every 6 hours or 1500 mg per day in divided doses. For hyoscine butylbromide, doses exceeding 100 mg per day are generally not recommended.
## Pediatric Dosing
Dosing for children is not well-established for this specific combination product. Mefenamic acid can be dosed in children aged 6 months and older (e.g., 5-7 mg/kg/dose every 6-8 hours, maximum 4 doses per day, not to exceed 200 mg per dose or 800 mg per day). Hyoscine butylbromide pediatric dosing is also variable and typically lower than adult doses. Consultation with a pediatrician is recommended.
## Dose Adjustments
* **Renal Impairment:** Use with caution in patients with moderate to severe renal impairment. Mefenamic acid can cause renal toxicity.
* **Hepatic Impairment:** Use with caution in patients with hepatic impairment.
* **Elderly:** Use with caution, particularly if frail or with comorbidities.
## Contraindications
* Hypersensitivity to mefenamic acid, hyoscine butylbromide, or any component of the formulation.
* Active or history of gastrointestinal ulceration or bleeding.
* Severe renal or hepatic impairment.
* Myasthenia gravis.
* Tachycardia.
* Glaucoma (narrow-angle).
* Prostatic hypertrophy with urinary retention.
* Ileus or intestinal obstruction.
* Megaloblastic anemia.
* Third trimester of pregnancy.
## Adverse Effects
* **Common:** Nausea, vomiting, diarrhea, constipation, abdominal pain, dizziness, headache, rash.
* **Serious:** Gastrointestinal bleeding, perforation, renal failure, hepatic dysfunction, Stevens-Johnson syndrome, anaphylaxis, blurred vision, urinary retention, tachycardia.
## Key Drug Interactions
* **NSAIDs:** Increased risk of gastrointestinal bleeding and renal toxicity when used concurrently with other NSAIDs, aspirin, or corticosteroids.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **ACE inhibitors, ARBs, diuretics:** Reduced antihypertensive effect and increased risk of renal impairment.
* **Lithium:** Mefenamic acid can increase serum lithium levels.
* **Methotrexate:** Increased methotrexate toxicity.
* **Potassium-sparing diuretics:** Increased risk of hyperkalemia.
* **Other anticholinergics:** Additive anticholinergic effects (e.g., increased dry mouth, constipation, blurred vision).
* **Dopamine antagonists:** May reduce the efficacy of prokinetic effects.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding (e.g., melena, hematemesis).
* Monitor renal function, especially in patients with pre-existing renal disease or those receiving concomitant nephrotoxic drugs.
* Monitor liver function tests if prolonged use is anticipated.
* Assess for signs of anticholinergic adverse effects.
## Clinical Pearls
* Meftal spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to the anticholinergic effects of hyoscine butylbromide, caution should be exercised in patients with conditions that can be exacerbated by these effects (e.g., prostatic hypertrophy, narrow-angle glaucoma, tachycardia).
* Avoid concurrent use with other NSAIDs.
* Short-term use is generally recommended to minimize risks.
