# Meftal spas
## Overview
Meftal spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and hyoscine butylbromide, an antispasmodic agent.
## Primary Indications
Meftal spas is indicated for the symptomatic relief of pain and spasms associated with gastrointestinal conditions, including irritable bowel syndrome, biliary colic, and renal colic.
## Adult Dosing
The typical adult dose is one tablet (containing 250 mg mefenamic acid and 10 mg hyoscine butylbromide) three times daily. Maximum daily dose of mefenamic acid is 1000 mg, and hyoscine butylbromide is 100 mg. Dosing may vary based on patient response and local protocols.
## Pediatric Dosing
Dosing for pediatric patients is not well-established. Mefenamic acid dosing in children is typically 5-7 mg/kg/dose every 6-8 hours. Hyoscine butylbromide is generally not recommended in children younger than 6 years.
## Dose Adjustments
No specific dose adjustments are typically required for hepatic or renal impairment, but caution is advised. Mefenamic acid should be used with caution in patients with pre-existing renal disease.
## Contraindications
* Known hypersensitivity to mefenamic acid, hyoscine butylbromide, or other NSAIDs.
* Active peptic ulceration or inflammatory bowel disease.
* Gastrointestinal bleeding.
* Severe hepatic or renal impairment.
* Myasthenia gravis.
* Tachyarrhythmias.
* Glaucoma.

## Adverse Effects
**Mefenamic Acid:** Gastrointestinal upset (nausea, vomiting, diarrhea, abdominal pain), dyspepsia, headache, dizziness, drowsiness, rash, renal impairment, bronchospasm, increased risk of cardiovascular thrombotic events.
**Hyoscine Butylbromide:** Anticholinergic effects such as dry mouth, blurred vision, constipation, urinary retention, tachycardia, drowsiness, confusion.
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **Other NSAIDs:** Additive effects, increased risk of adverse events.
* **Diuretics and antihypertensives:** NSAIDs may reduce their efficacy and increase risk of renal toxicity.
* **Lithium:** NSAIDs can increase lithium levels.
* **Methotrexate:** NSAIDs can increase methotrexate toxicity.
* **Antacids:** May decrease absorption of hyoscine butylbromide.
* **Amanadine, quinidine, tricyclic antidepressants, antihistamines:** Increased anticholinergic effects.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding or ulceration.
* Assess renal function, especially in patients with pre-existing renal disease or those on concurrent nephrotoxic agents.
* Monitor for signs of cardiovascular events.
* Monitor for anticholinergic side effects.
## Clinical Pearls
* Meftal spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to anticholinergic effects, caution should be exercised when driving or operating machinery.
* Prolonged use of mefenamic acid may lead to renal damage.
* The combination is generally used for short-term symptomatic relief.
