# Meftal spas
## Overview
Meftal spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and dicyclomine, an anticholinergic antispasmodic. Mefenamic acid works by inhibiting prostaglandin synthesis, while dicyclomine reduces smooth muscle activity in the gastrointestinal tract.
## Primary Indications
* Abdominal pain associated with smooth muscle spasms, particularly when inflammation is also present.
## Adult Dosing
* **Usual Dose:** 1 tablet (mefenamic acid 250 mg / dicyclomine 20 mg) every 8 hours as needed.
* **Maximum Dose:** Not explicitly defined, but prolonged use and higher doses of mefenamic acid increase the risk of adverse effects. Dicyclomine dose should not exceed 160 mg daily.
## Pediatric Dosing
* Not recommended for use in children under 12 years of age due to lack of established safety and efficacy data.

* **Serious:** Gastrointestinal bleeding, ulceration, perforation; renal toxicity; hepatic toxicity; exacerbation of asthma; cardiovascular thrombotic events (with NSAIDs); blurred vision, urinary retention, constipation, confusion (with dicyclomine).
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **Diuretics, ACE inhibitors, ARBs:** May reduce antihypertensive effects and increase risk of renal impairment.
* **Lithium:** Mefenamic acid can increase serum lithium levels.
* **Other NSAIDs/Aspirin:** Increased risk of gastrointestinal adverse effects.
* **Anticholinergic agents:** Additive anticholinergic effects.
## Monitoring
* **Renal function:** Especially in patients with pre-existing renal disease or those at risk.
* **Liver function:** Particularly with long-term use or in patients with hepatic impairment.
* **Signs of gastrointestinal bleeding:** Stool guaiac, symptoms like melena or hematemesis.
* **Blood pressure:** NSAIDs can cause fluid retention and hypertension.
## Clinical Pearls
* Meftal spas should be used for the shortest duration necessary to control symptoms.
* Administer with food or milk to minimize gastrointestinal upset.
* Caution patients about operating machinery or driving until they know how the medication affects them, due to potential dizziness and drowsiness.
* The anticholinergic effects of dicyclomine can be exacerbated by other medications with anticholinergic properties.
