# Meftal spas
## Overview
Meftal spas is a combination product containing **mefenamic acid**, a nonsteroidal anti-inflammatory drug (NSAID), and **hyoscine butylbromide** (also known as scopolamine butylbromide), an antispasmodic agent.
## Primary Indications
* Symptomatic relief of abdominal pain and cramps associated with gastrointestinal spasms.
## Adult Dosing
* **Mefenamic acid 250 mg / Hyoscine butylbromide 10 mg:**
* 1 tablet orally every 8 hours.
* Maximum: 3 tablets per day.
* Duration of use should generally not exceed 3-5 days.
## Pediatric Dosing
* Information regarding pediatric use of Meftal spas is not well-established. Use with caution and consider age-appropriate NSAID and antispasmodic alternatives. Consult specific pediatric guidelines.
## Dose Adjustments
* **Renal Impairment:** Mefenamic acid should be used with caution. Monitor renal function.
* **Hepatic Impairment:** Mefenamic acid should be used with caution. Monitor liver function.
* **Elderly:** Increased risk of adverse effects associated with NSAIDs. Use with caution and consider lower doses.
## Contraindications
* Hypersensitivity to mefenamic acid, hyoscine butylbromide, or any component of the formulation.
* Active gastrointestinal ulceration or inflammation.
* History of NSAID-induced bronchospasm, urticaria, or allergic rhinitis.
* Severe renal or hepatic impairment.
* Myasthenia gravis.
* Tachyarrhythmias.
* Angle-closure glaucoma.
* Obstructive uropathy or gastrointestinal obstruction.
* Prostatic hypertrophy with urinary retention.

## Adverse Effects
* **Common (Mefenamic Acid):** Gastrointestinal upset (nausea, vomiting, diarrhea, abdominal pain), headache, dizziness, drowsiness.
* **Common (Hyoscine Butylbromide):** Dry mouth, blurred vision, constipation, urinary retention, tachycardia.
* **Serious:** Gastrointestinal bleeding, perforation, renal failure, hepatic impairment, exacerbation of asthma, cardiovascular events (associated with NSAIDs). Allergic reactions.
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **Other NSAIDs:** Increased risk of adverse effects.
* **SSRIs/SNRIs:** Increased risk of gastrointestinal bleeding.
* **Diuretics, ACE inhibitors, ARBs:** Reduced antihypertensive effect and increased risk of renal impairment.
* **Digoxin:** Increased digoxin levels.
* **Methotrexate:** Increased methotrexate toxicity.
* **Anticholinergic agents:** Additive anticholinergic effects.
* **Dopamine antagonists:** May reduce the efficacy of dopamine antagonists.
* **Beta-agonists:** May reduce the bronchodilator effect of beta-agonists.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding.
* Monitor renal and hepatic function, especially in patients with pre-existing impairment or in the elderly.
* Monitor for signs of anticholinergic adverse effects.
* Assess pain relief and symptom control.
## Clinical Pearls
* Meftal spas should be used for the shortest duration necessary to control symptoms.
* Avoid concomitant use with alcohol due to increased risk of gastrointestinal side effects.
* Patients with a history of asthma, allergies, or gastrointestinal disorders should be monitored closely.
* Due to the anticholinergic effects of hyoscine butylbromide, caution is advised in patients with conditions that can be exacerbated by these effects (e.g., heart disease, hyperthyroidism).
