# Meftal spas
## Overview
Meftal spas is a combination product containing mefenamic acid, a non-steroidal anti-inflammatory drug (NSAID), and dicyclomine, an anticholinergic antispasmodic. Mefenamic acid works by inhibiting prostaglandin synthesis, reducing pain and inflammation. Dicyclomine reduces gastrointestinal smooth muscle motility and spasms.
## Primary Indications
* Symptomatic relief of abdominal pain and cramps associated with irritable bowel syndrome (IBS).
* Relief of spasmodic abdominal pain.
## Adult Dosing
* **Meftal spas:** One tablet (containing mefenamic acid 250 mg and dicyclomine hydrochloride 10 mg) three times daily after food.
* **Maximum:** Not explicitly defined, but generally limit NSAID use to the lowest effective dose for the shortest duration necessary. Dicyclomine doses typically do not exceed 80 mg per day.
## Pediatric Dosing
* Safety and efficacy in pediatric patients have not been established. Use is not recommended.
## Dose Adjustments
* **Renal Impairment:** Mefenamic acid is contraindicated in severe renal impairment. Caution and potential dose reduction may be considered in mild to moderate impairment, though specific guidelines are lacking.
* **Hepatic Impairment:** Mefenamic acid is contraindicated in severe hepatic impairment. Caution and potential dose reduction may be considered in mild to moderate impairment, though specific guidelines are lacking.
* **Elderly:** Increased risk of adverse effects, particularly gastrointestinal bleeding and renal events. Use with caution and at the lowest effective dose.
## Contraindications
* Known hypersensitivity to mefenamic acid, dicyclomine, aspirin, or other NSAIDs.
* History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Severe renal impairment.
* Severe hepatic impairment.
* Severe ulcerative colitis.
* Gastrointestinal bleeding or perforation.
* Obstructive uropathy.
* Uncontrolled glaucoma.
* Myasthenia gravis.
* Infants less than 6 months of age.
* Pregnancy (especially third trimester).
## Adverse Effects
* **Common:** Nausea, vomiting, diarrhea, abdominal pain, constipation, dyspepsia, dizziness, drowsiness, dry mouth.
* **Serious (Mefenamic Acid):** Gastrointestinal bleeding, ulceration, perforation; renal impairment; cardiovascular thrombotic events; hepatic reactions; hypersensitivity reactions.
* **Serious (Dicyclomine):** Anticholinergic effects (blurred vision, dilated pupils, tachycardia, urinary retention, constipation, confusion, drowsiness, hallucinations), especially in the elderly or those with underlying conditions.
## Key Drug Interactions
* **NSAIDs (other):** Increased risk of gastrointestinal bleeding and renal toxicity.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal bleeding.
* **SSRIs/SNRIs:** Increased risk of gastrointestinal bleeding.
* **Diuretics, ACE inhibitors, ARBs:** Reduced antihypertensive efficacy and increased risk of renal impairment.
* **Lithium:** Increased serum lithium levels.
* **Methotrexate:** Increased methotrexate toxicity.
* **Anticholinergics (other):** Additive anticholinergic effects.
* **Antacids:** May decrease absorption of mefenamic acid.
## Monitoring
* **Renal function:** Baseline and periodic monitoring, especially in patients with pre-existing renal disease or those on concurrent nephrotoxic agents.
* **Hepatic function:** Baseline and periodic monitoring, especially in patients with pre-existing hepatic disease.
* **Gastrointestinal symptoms:** Monitor for signs of bleeding (melena, hematemesis) or ulceration.
* **Blood pressure:** Monitor for potential increases, especially in patients with hypertension.
* **Signs of anticholinergic toxicity:** Especially in elderly patients or those on other anticholinergic medications.
## Clinical Pearls
* Meftal spas should be used cautiously due to the potential for serious gastrointestinal, renal, and cardiovascular events associated with NSAIDs, and anticholinergic effects from dicyclomine.
* Administer with food or milk to minimize gastrointestinal upset.
* Avoid concomitant use with other NSAIDs.
* Use dicyclomine component with caution in patients with prostatic hypertrophy, hiatal hernia, or reflux esophagitis.
* Due to anticholinergic effects, caution patients regarding activities requiring mental alertness (e.g., operating machinery, driving).
