# Meftal Spas
## Overview
Meftal Spas is a combination medication containing mefenamic acid and dicyclomine hydrochloride. Mefenamic acid is a nonsteroidal anti-inflammatory drug (NSAID) used for mild to moderate pain. Dicyclomine hydrochloride is an anticholinergic and antispasmodic agent used to relieve smooth muscle spasms.
## Primary Indications
* Pain associated with abdominal cramps and spasms, particularly in gynecological conditions (e.g., dysmenorrhea).
* Relief of mild to moderate pain.
## Adult Dosing
* **Meftal Spas Tablets:** Typically 1 tablet (containing mefenamic acid 250 mg and dicyclomine hydrochloride 20 mg) three times daily after food.
* **Meftal Spas Suspension:** Dosage varies by concentration. Follow product labeling or local protocol. A common regimen might be 10 mL three times daily after food.
* Maximum daily dose for mefenamic acid is generally 1500 mg. Maximum daily dose for dicyclomine is generally 80 mg.
## Pediatric Dosing
* Dosing for children is not well-established and should be based on age, weight, and clinical assessment. Pediatric use of mefenamic acid and dicyclomine should be approached with caution. Consult product labeling or pediatric pharmacotherapy resources.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is primarily eliminated by the kidneys. Moderate to severe impairment may require dose reduction or avoidance.
* **Hepatic Impairment:** Use with caution. Mefenamic acid is metabolized by the liver.
* **Elderly:** Increased risk of adverse effects from NSAIDs, including gastrointestinal bleeding and renal impairment. Consider lower doses and close monitoring.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or other NSAIDs.
* Patients with a history of bronchospasm, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Inflammatory bowel disease.
* Obstructive uropathy or gastrointestinal obstruction.
* Severe renal or hepatic impairment.
* Uncontrolled heart failure.
* History of gastrointestinal bleeding or perforation related to previous NSAID use.
* Active peptic ulceration or history of recurrent peptic ulceration.
* Third trimester of pregnancy.
* Glaucoma (dicyclomine may worsen angle-closure glaucoma).
* Infants less than 6 months of age (increased risk of respiratory depression and other side effects with dicyclomine).
## Adverse Effects
* **Gastrointestinal:** Nausea, vomiting, diarrhea, abdominal pain, dyspepsia, gastrointestinal bleeding, ulceration, perforation.
* **Central Nervous System:** Dizziness, drowsiness, headache, nervousness, confusion (especially in elderly).
* **Anticholinergic Effects (Dicyclomine):** Dry mouth, blurred vision, urinary retention, constipation, decreased sweating, tachycardia.
* **Other:** Rash, pruritus, bronchospasm, renal impairment, hypertension, fluid retention.
## Key Drug Interactions
* **NSAIDs:** Increased risk of bleeding and renal toxicity when combined with other NSAIDs, aspirin, or anticoagulants (e.g., warfarin).
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **Diuretics, ACE Inhibitors, ARBs:** Reduced antihypertensive effect and increased risk of renal impairment.
* **Lithium:** Increased serum lithium levels.
* **Methotrexate:** Increased risk of methotrexate toxicity.
* **Anticholinergics:** Additive anticholinergic effects with other agents (e.g., tricyclic antidepressants, antihistamines).
* **Antacids/Cholestyramine:** May reduce absorption of mefenamic acid.
## Monitoring
* **Renal Function:** Baseline and periodic monitoring of serum creatinine and BUN, especially in patients with risk factors.
* **Gastrointestinal Bleeding:** Monitor for signs and symptoms of bleeding (e.g., melena, hematemesis, abdominal pain).
* **Hepatic Function:** Baseline and periodic monitoring of LFTs, especially in patients with hepatic impairment.
* **Blood Pressure:** Monitor blood pressure, particularly in patients with hypertension.
* **Signs of Anticholinergic Effects:** Monitor for dry mouth, blurred vision, urinary retention.
## Clinical Pearls
* Administer with food or milk to minimize gastrointestinal upset.
* Avoid concurrent use with other NSAIDs.
* Due to anticholinergic effects, caution is advised in patients with conditions like myasthenia gravis, prostatic hypertrophy, or in hot weather.
* Dicyclomine's use in infants is generally discouraged due to potential for serious adverse effects.
* NSAIDs carry a risk of serious cardiovascular thrombotic events, myocardial infarction, and stroke.
