# Meftal Spas
## Overview
Meftal Spas is a combination product containing **mefenamic acid**, a nonsteroidal anti-inflammatory drug (NSAID), and **dicyclomine hydrochloride**, an anticholinergic antispasmodic.
## Primary Indications
* Symptomatic relief of mild to moderate pain associated with smooth muscle spasm, particularly in the gastrointestinal tract.
* Painful menstruation (dysmenorrhea).
## Adult Dosing
* **Mefenamic Acid Component:** Typically 250 mg to 500 mg orally every 8 hours as needed.
* **Dicyclomine Hydrochloride Component:** Typically 20 mg orally every 8 hours as needed.
* *Note:* The combination product usually provides these doses in a single tablet or capsule. The maximum duration of use for symptomatic relief is generally limited. Consult specific product labeling or local protocol for exact dosing and duration.
## Pediatric Dosing
* Dosing in pediatric patients is not well-established and generally not recommended due to risks associated with NSAIDs and anticholinergics in this population. Consult a pediatrician for alternative management strategies.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is renally excreted. Dose reduction or avoidance may be necessary in severe renal impairment.
* **Hepatic Impairment:** Use with caution. Mefenamic acid is metabolized in the liver. Dose reduction or avoidance may be necessary in severe hepatic impairment.

## Contraindications

## Adverse Effects

* **Renal:** Renal papillary necrosis, interstitial nephritis, renal failure.
* **Hepatic:** Hepatotoxicity, elevated liver enzymes.
* **Anticholinergic:** Blurred vision, urinary retention, tachycardia, confusion (especially in the elderly).
* **Hematologic:** Anemia.
* **Hypersensitivity:** Severe skin reactions (e.g., Stevens-Johnson syndrome, toxic epidermal necrolysis).
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **Other NSAIDs/Aspirin:** Increased risk of gastrointestinal adverse effects.
* **ACE Inhibitors/ARBs/Diuretics:** May reduce antihypertensive effects and increase risk of renal impairment.
* **Lithium:** Increased lithium levels.
* **Methotrexate:** Increased methotrexate toxicity.
* **Anticholinergics:** Additive anticholinergic effects.
* **CNS Depressants:** Additive sedative effects.
## Monitoring
* **Renal function:** Baseline and periodic monitoring, especially in patients with risk factors.
* **Liver function:** Baseline and periodic monitoring.
* **Gastrointestinal symptoms:** Monitor for signs of bleeding or ulceration.
* **Blood pressure:** Especially in patients on antihypertensives.
* **Signs of anticholinergic toxicity:** Particularly in the elderly.
## Clinical Pearls
* Use the lowest effective dose for the shortest duration necessary.
* Meftal Spas should be taken with food or milk to minimize gastrointestinal upset.
* Avoid concomitant use with other NSAIDs, aspirin, or alcohol.
* Patients should be advised of the potential for drowsiness and impaired ability to operate machinery or drive.
* Discontinue immediately if signs of gastrointestinal bleeding (e.g., black, tarry stools, coffee-ground emesis) or severe allergic reaction occur.
