# Meftal Spas
## Overview
Meftal Spas is a combination product containing Mefenamic Acid (a nonsteroidal anti-inflammatory drug - NSAID) and Dicyclomine Hydrochloride (an anticholinergic antispasmodic).
## Primary Indications
* Management of mild to moderate pain associated with smooth muscle spasms, particularly in conditions like dysmenorrhea.
## Adult Dosing
* **Mefenamic Acid:** 500 mg every 8 hours as needed for pain. Maximum daily dose is 1500 mg.
* **Dicyclomine Hydrochloride:** 20 mg every 8 hours as needed for spasms.
*Note: Specific dosing regimens may vary based on local protocol and patient response.*
## Pediatric Dosing
* Mefenamic Acid: 6.5 mg/kg/dose every 8 hours, not to exceed 500 mg per dose or 1500 mg per day.
* Dicyclomine Hydrochloride: Dosing is not well-established in pediatric populations. Use with caution and close monitoring if deemed necessary.
## Dose Adjustments
* **Renal Impairment:** Use with caution. NSAIDs can worsen renal function.
* **Hepatic Impairment:** Use with caution. Monitor liver function.
* **Elderly:** Increased risk of adverse effects, particularly gastrointestinal bleeding and renal toxicity. Lower doses may be considered.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or other NSAIDs.
* History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Inflammatory bowel disease.
* Obstructive uropathy or gastrointestinal obstruction.
* Severe renal or hepatic impairment.
* Myasthenia gravis.
* Glaucoma.
* Infants less than 6 months of age (due to dicyclomine).
* Use in the elderly (caution advised).
## Adverse Effects
* **Mefenamic Acid:** Gastrointestinal (dyspepsia, nausea, vomiting, diarrhea, constipation, GI bleeding, ulceration), renal toxicity, hypersensitivity reactions.
* **Dicyclomine Hydrochloride:** Anticholinergic effects (dry mouth, blurred vision, urinary retention, constipation, drowsiness, dizziness, confusion).
* **Both:** Headache, dizziness, rash.
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Other NSAIDs/Aspirin:** Increased risk of GI toxicity and bleeding.
* **ACE inhibitors/ARBs/Diuretics:** Reduced antihypertensive effect and increased risk of renal impairment.
* **Corticosteroids:** Increased risk of GI toxicity.
* **Lithium:** Increased lithium levels.
* **Methotrexate:** Increased methotrexate toxicity.
* **Anticholinergic agents:** Additive anticholinergic effects.
* **Antacids:** May affect absorption of mefenamic acid.
## Monitoring
* **Renal function:** Baseline and periodically, especially in patients with pre-existing renal disease or those at risk.
* **Hepatic function:** Baseline and periodically, especially in patients with pre-existing hepatic disease.
* **Signs of GI bleeding:** Monitor for melena, hematemesis, abdominal pain.
* **Blood pressure:** Monitor for changes.
* **Signs of anticholinergic toxicity:** Especially in elderly patients or those taking other anticholinergic medications.
## Clinical Pearls
* Meftal Spas should be taken with food or milk to minimize GI upset.
* Duration of therapy should be as short as possible.
* Due to the dicyclomine component, patients should be advised about potential anticholinergic side effects, especially if operating machinery or driving.
* Avoid concurrent use with other NSAIDs.
