# Meftal Spas
## Overview
Meftal Spas is a combination medication containing mefenamic acid (a nonsteroidal anti-inflammatory drug, NSAID) and dicyclomine hydrochloride (an anticholinergic antispasmodic). Mefenamic acid reduces inflammation and pain, while dicyclomine relaxes smooth muscles in the gastrointestinal tract, relieving spasms.
## Primary Indications
* Symptomatic relief of moderate to severe pain associated with smooth muscle spasms, particularly in gynecological conditions and gastrointestinal disorders.
## Adult Dosing
* **Meftal Spas Tablet:** Typically, 1 tablet every 8 hours as needed for pain. Maximum daily dose is usually 4 tablets.
* **Meftal Spas Suspension:** Refer to product labeling for specific pediatric and adult dosing based on weight and age.
## Pediatric Dosing
* Pediatric dosing for Meftal Spas is not well-established and should be determined by a healthcare provider based on age, weight, and clinical indication. Mefenamic acid itself has pediatric dosing guidelines, but the combination with dicyclomine requires careful consideration.
* For mefenamic acid alone in pediatrics (ages 6 months and older): 6.5 mg/kg every 8 hours. Maximum daily dose should not exceed 500 mg.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is renally excreted. Dose reduction may be necessary in severe renal impairment.
* **Hepatic Impairment:** Use with caution. Mefenamic acid is metabolized by the liver.
* **Elderly:** Increased risk of adverse effects, particularly gastrointestinal bleeding, renal, and cardiovascular events. Use cautiously and at the lowest effective dose.
## Contraindications
* Known hypersensitivity to mefenamic acid, dicyclomine, or other NSAIDs.
* Patients with a history of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Inflammatory bowel disease (e.g., Crohn's disease, ulcerative colitis).
* Gastrointestinal obstruction or severe ulcerative colitis.
* Myasthenia gravis.
* Glaucoma (narrow-angle).
* Obstructive uropathy.
* Uncontrolled heart failure.
* Severe renal or hepatic impairment.
* Pregnancy (especially third trimester).
## Adverse Effects
* **Common (Mefenamic Acid):** Diarrhea, nausea, vomiting, abdominal pain, dyspepsia, constipation.
* **Common (Dicyclomine):** Dry mouth, blurred vision, constipation, urinary retention, dizziness, drowsiness.
* **Serious:** Gastrointestinal bleeding, ulceration, perforation; renal papillary necrosis; hepatitis; hypersensitivity reactions (including anaphylaxis); exacerbation of asthma; cardiovascular thrombotic events; central nervous system effects (confusion, hallucinations).
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **Other NSAIDs (including aspirin):** Increased risk of gastrointestinal toxicity and bleeding.
* **Diuretics, ACE inhibitors, ARBs:** Reduced antihypertensive effect and increased risk of renal impairment.
* **Lithium:** Increased serum lithium levels.
* **Anticholinergics (e.g., atropine, antihistamines):** Additive anticholinergic effects.
* **Metoclopramide:** May reduce the efficacy of metoclopramide.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding (e.g., melena, hematemesis).
* Assess renal function (serum creatinine, BUN) periodically, especially in patients with pre-existing renal disease or those on concomitant nephrotoxic agents.
* Monitor liver function tests periodically.
* Assess for efficacy and adverse effects, particularly anticholinergic side effects.
## Clinical Pearls
* This combination is typically used for short-term management of pain associated with spasms.
* Due to the anticholinergic component, caution is advised in the elderly and in patients with conditions that can be exacerbated by anticholinergics (e.g., prostatic hypertrophy, tachycardia).
* Mefenamic acid should be used at the lowest effective dose and for the shortest duration necessary to minimize risks of serious cardiovascular, gastrointestinal, and renal adverse events.
* Always counsel patients on potential side effects and when to seek medical attention.
