# Meftal Spas
## Overview
Meftal Spas is a combination medication containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and hyoscine butylbromide, an antispasmodic. Mefenamic acid reduces inflammation and pain by inhibiting prostaglandin synthesis. Hyoscine butylbromide acts peripherally to reduce smooth muscle spasms, particularly in the gastrointestinal tract.
## Primary Indications
* Management of abdominal pain associated with spasms, such as in irritable bowel syndrome (IBS), diverticulitis, and other gastrointestinal motility disorders.
* Relief of pain and inflammation associated with dysmenorrhea.
## Adult Dosing
* **Abdominal Spasms/Pain:** 1 tablet (mefenamic acid 250 mg, hyoscine butylbromide 10 mg) orally every 8 hours as needed. Maximum daily dose: 4 tablets.
* **Dysmenorrhea:** 1-2 tablets (mefenamic acid 250 mg, hyoscine butylbromide 10 mg) orally every 8 hours as needed, starting with the onset of pain and continuing for up to 3 days.
## Pediatric Dosing
The use of Meftal Spas in pediatric patients is generally not recommended due to lack of established safety and efficacy data. Specific pediatric dosing for mefenamic acid can be found in pediatric drug references, but combination products like Meftal Spas are typically reserved for adults.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is renally excreted. Dose reduction may be considered in severe renal impairment, though specific guidelines are lacking.
* **Hepatic Impairment:** Use with caution. Mefenamic acid is metabolized in the liver. Dose reduction may be considered in severe hepatic impairment, though specific guidelines are lacking.
* **Elderly:** Elderly patients are at increased risk of adverse effects from NSAIDs, including gastrointestinal bleeding and renal impairment. Use with caution and at the lowest effective dose.
## Contraindications
* Hypersensitivity to mefenamic acid, hyoscine butylbromide, or other NSAIDs.
* Active peptic ulceration or a history of recurrent peptic ulceration.
* Severe renal or hepatic impairment.
* History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Intestinal obstruction or paralytic ileus.
* Glaucoma (relative contraindication for hyoscine component).
* Myasthenia gravis (relative contraindication for hyoscine component).
* Prostatic hypertrophy with urinary retention (relative contraindication for hyoscine component).
* Tachyarrhythmias.
## Adverse Effects
* **Common (Mefenamic Acid):** Gastrointestinal upset (nausea, vomiting, diarrhea, abdominal pain, dyspepsia), headache, dizziness, rash.
* **Less Common (Mefenamic Acid):** Gastrointestinal bleeding, peptic ulceration, renal impairment, elevated liver enzymes, edema, hypertension, confusion, drowsiness.
* **Common (Hyoscine Butylbromide):** Dry mouth, blurred vision, constipation, urinary retention, increased heart rate.
* **Rare:** Allergic reactions.
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **Other NSAIDs/Aspirin:** Increased risk of gastrointestinal adverse effects and potentially additive renal effects.
* **ACE Inhibitors/ARBs/Diuretics:** May reduce antihypertensive effect and increase risk of renal impairment.
* **Lithium:** NSAIDs can increase lithium levels.
* **Methotrexate:** NSAIDs can increase methotrexate toxicity.
* **Anticholinergics:** Additive anticholinergic effects (e.g., with antihistamines, tricyclic antidepressants).
* **Dopamine antagonists (e.g., metoclopramide):** May antagonize the effects of hyoscine.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding (e.g., melena, hematemesis, abdominal pain).
* Monitor renal and hepatic function, especially in patients with pre-existing impairment or those on long-term therapy.
* Monitor blood pressure, particularly in patients with hypertension.
* Assess for efficacy and adverse effects regularly.
## Clinical Pearls
* Meftal Spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to the anticholinergic effects of hyoscine, patients should be advised about potential side effects like blurred vision and dry mouth, and cautioned when driving or operating machinery.
* Avoid concurrent use with other NSAIDs.
* Duration of treatment should be as short as possible and at the lowest effective dose.
