# Meftal Spas
## Overview
Meftal Spas is a combination medication containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and dicyclomine hydrochloride, an anticholinergic antispasmodic.
## Primary Indications
* Symptomatic relief of abdominal pain associated with smooth muscle spasm, particularly in conditions like irritable bowel syndrome, diverticulitis, and functional bowel disorders.
* Management of primary dysmenorrhea.
## Adult Dosing
* **Abdominal Pain/Spasm:** 1 capsule (250 mg mefenamic acid / 20 mg dicyclomine) every 8 hours as needed. Maximum dose: Not well established for this specific combination, but mefenamic acid alone should not exceed 1000 mg every 8 hours or 3000 mg/day. Dicyclomine alone typically does not exceed 80 mg/day.
* **Dysmenorrhea:** 1 capsule every 8 hours, usually starting with the onset of pain and continuing for 2-3 days.
## Pediatric Dosing
Specific dosing for Meftal Spas in pediatric patients is not established. Mefenamic acid dosing in children typically starts at 6.5 mg/kg every 6 hours. Dicyclomine has limited use in children and requires careful consideration of risks. Consult with a pediatrician or refer to pediatric-specific guidelines.
## Dose Adjustments

## Adverse Effects
* **Mefenamic Acid:** Gastrointestinal upset, nausea, vomiting, diarrhea, constipation, abdominal pain, dyspepsia, rash, dizziness, headache, tinnitus, renal dysfunction, fluid retention, increased risk of cardiovascular thrombotic events, myocardial infarction, and stroke. Gastrointestinal bleeding and perforation are serious risks.
* **Dicyclomine:** Dry mouth, blurred vision, urinary retention, constipation, drowsiness, confusion, dizziness, nervousness, nausea, skin rash. Increased risk in elderly.
## Key Drug Interactions
* **Other NSAIDs/Aspirin:** Increased risk of GI toxicity.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of GI bleeding.
* **SSRIs:** Increased risk of GI bleeding.
* **Diuretics, ACE inhibitors, ARBs:** May reduce antihypertensive effect and increase risk of renal impairment.
* **Lithium:** Mefenamic acid can increase lithium levels.
* **Anticholinergics (e.g., antihistamines, TCAs):** Additive anticholinergic effects with dicyclomine.
* **Antacids:** May decrease absorption of dicyclomine.
## Monitoring
* **Renal function:** Monitor serum creatinine and BUN, especially in patients with pre-existing renal disease or those at risk.
* **Hepatic function:** Monitor liver enzymes periodically, especially with long-term use.
* **Gastrointestinal symptoms:** Educate patients on signs of GI bleeding (e.g., black tarry stools, hematemesis) and to report them immediately.
* **Blood pressure:** Monitor for potential increases.

* Meftal Spas combines an anti-inflammatory (mefenamic acid) with an antispasmodic (dicyclomine) for dual action in pain relief.
* Due to the risk of gastrointestinal bleeding with NSAIDs, use the lowest effective dose for the shortest duration necessary.
