# Meftal spas
## Overview
Meftal spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and hyoscine butylbromide, an anticholinergic antispasmodic agent.
## Primary Indications
* Symptomatic relief of abdominal pain and spasms associated with gastrointestinal disorders, including irritable bowel syndrome and biliary colic.
* Primary dysmenorrhea.
## Adult Dosing
* **Abdominal Pain/Spasms:** 1 tablet (mefenamic acid 250 mg/hyoscine butylbromide 10 mg) three times daily. Maximum dose: Not established for the combination. For mefenamic acid alone, typically 500 mg every 6-8 hours. For hyoscine butylbromide alone, typically 10-20 mg three times daily.
* **Dysmenorrhea:** 1 tablet (mefenamic acid 250 mg/hyoscine butylbromide 10 mg) three times daily, starting with the onset of pain and continuing for up to 3 days.
## Pediatric Dosing
* Mefenamic acid pediatric dosing is established, but hyoscine butylbromide pediatric dosing is not well-established for this combination product. **Use in pediatrics is generally not recommended due to lack of established safety and efficacy data for the combination.** For mefenamic acid alone, pediatric doses vary by age and indication.
## Dose Adjustments
* **Renal Impairment:** Mefenamic acid should be used with caution in patients with severe renal impairment. Dose reduction may be necessary, though specific guidelines for the combination are not available.
* **Hepatic Impairment:** Mefenamic acid should be used with caution in patients with hepatic impairment. Dose reduction may be necessary, though specific guidelines for the combination are not available.

## Contraindications
* Hypersensitivity to mefenamic acid, hyoscine butylbromide, or any excipient.

* History of bronchospasm.

## Adverse Effects
* **Mefenamic Acid:** Gastrointestinal upset (nausea, vomiting, diarrhea, constipation, dyspepsia), abdominal pain, headache, dizziness, rash, Stevens-Johnson syndrome, toxic epidermal necrolysis, renal impairment, hepatic dysfunction, increased risk of cardiovascular thrombotic events.
* **Hyoscine Butylbromide:** Dry mouth, blurred vision, constipation, urinary retention, tachycardia, drowsiness, confusion (especially in elderly).
## Key Drug Interactions
* **Mefenamic Acid:**
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **SSRIs/SNRIs:** Increased risk of gastrointestinal bleeding.
* **Diuretics, ACE inhibitors, ARBs:** Reduced antihypertensive effect and increased risk of renal impairment.
* **Lithium:** Increased lithium levels.
* **Methotrexate:** Increased methotrexate toxicity.
* **Hyoscine Butylbromide:**
* **Other anticholinergics:** Additive effects, increasing risk of adverse effects.
* **Dopamine antagonists (e.g., metoclopramide):** Antagonistic effects on gastrointestinal motility.
* **Opioids:** Increased risk of severe constipation and ileus.
## Monitoring
* **Renal and hepatic function:** Especially in patients with pre-existing impairment or those on long-term therapy.
* **Gastrointestinal symptoms:** Monitor for signs of bleeding or ulceration.
* **Cardiovascular status:** Monitor for signs of thrombotic events.
* **Urinary retention:** Monitor in susceptible individuals.
* **Vision:** Monitor for blurred vision.
## Clinical Pearls
* Meftal spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to the anticholinergic component, caution is advised in patients with glaucoma, prostatic hypertrophy, or cardiac arrhythmias.
* The combination is generally not recommended for chronic use due to the risks associated with NSAIDs.
* Consider the individual components' risks and benefits, especially in specific patient populations.
