# Meftal spas
## Overview
Meftal spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and dicyclomine hydrochloride, an anticholinergic antispasmodic.
## Primary Indications
* Symptomatic relief of painful conditions associated with smooth muscle spasms, particularly in the gastrointestinal and genitourinary tracts.
* Relief of mild to moderate pain.
## Adult Dosing
* **Meftal spas:** Typically 1 tablet (mefenamic acid 250 mg, dicyclomine HCl 20 mg) every 8 hours as needed.
* **Maximum dose:** Consult prescribing information. For mefenamic acid, typically limited to 7 days for acute pain and 10 days for chronic pain. For dicyclomine, maximum daily dose is generally 80 mg.
## Pediatric Dosing
* Safety and efficacy in pediatric patients are not well-established. Dosing should be determined by a healthcare provider based on patient weight and clinical condition.
## Dose Adjustments
* **Renal impairment:** Use with caution. Mefenamic acid is renally excreted.
* **Hepatic impairment:** Use with caution.
* **Elderly:** Increased risk of adverse effects, particularly gastrointestinal bleeding and renal dysfunction. Consider lower doses and shorter durations of therapy.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or other NSAIDs.
* History of asthma, urticaria, or allergic-type reactions after aspirin or other NSAIDs.
* Intestinal obstruction, paralytic ileus, severe ulcerative colitis, toxic megacolon.
* Myasthenia gravis.
* Uncontrolled glaucoma.
* Severe renal or hepatic impairment.
* Third trimester of pregnancy.
## Adverse Effects
* **Mefenamic acid:** Gastrointestinal disturbances (dyspepsia, nausea, vomiting, diarrhea, constipation, abdominal pain), bleeding, ulceration, perforation. Renal toxicity, hepatic toxicity, hypersensitivity reactions (rash, asthma), headache, dizziness.
* **Dicyclomine:** Anticholinergic effects (dry mouth, blurred vision, urinary retention, constipation, drowsiness, confusion, dizziness).
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Other NSAIDs and Aspirin:** Increased risk of GI toxicity.
* **Corticosteroids:** Increased risk of GI bleeding.
* **Diuretics, ACE inhibitors, ARBs:** May reduce antihypertensive effect and increase risk of renal impairment.
* **Lithium:** Increased lithium levels.
* **Methotrexate:** Increased methotrexate levels.
* **Anticholinergics:** Additive anticholinergic effects.
* **Antacids:** May alter absorption.
## Monitoring
* **Renal function:** Baseline and periodic monitoring, especially in elderly or those with pre-existing renal disease.
* **Hepatic function:** Baseline and periodic monitoring.
* **Gastrointestinal bleeding:** Monitor for signs and symptoms (e.g., melena, hematemesis).
* **Blood pressure:** Monitor, especially when used with antihypertensives.
## Clinical Pearls
* Use the lowest effective dose for the shortest duration necessary to minimize risks.
* Meftal spas should be taken with food or milk to reduce gastric irritation.
* Avoid concomitant use of other NSAIDs.
* Caution in patients with pre-existing GI disease, cardiovascular disease, or risk factors for these conditions.
* Anticholinergic effects of dicyclomine can be exacerbated by other medications with similar properties.
