# Meftal Spas
## Overview
Meftal Spas is a combination product containing mefenamic acid (a nonsteroidal anti-inflammatory drug, NSAID) and dicyclomine hydrochloride (an anticholinergic antispasmodic). Mefenamic acid reduces inflammation and pain. Dicyclomine reduces spasms in the gastrointestinal tract.
## Primary Indications
* Symptomatic treatment of abdominal pain associated with spasmodic conditions of the gastrointestinal tract, particularly when inflammation may also be present.
## Adult Dosing
* **Meftal Spas Tablet:** Typically, 1 tablet (250 mg mefenamic acid / 20 mg dicyclomine hydrochloride) three times daily, or as directed by a physician.
* **Meftal Spas Suspension:** Dosage is individualized based on age, weight, and severity of symptoms. A common pediatric dose is 0.5 mL/kg every 6-8 hours, not to exceed adult doses. Refer to product labeling or local protocol for specific pediatric dosing.
## Pediatric Dosing
* Dosing in children is typically based on weight and severity. Precise pediatric dosing may vary. Consult the specific product monograph or local guidelines for pediatric dosing recommendations. Generally, it is not recommended for infants under 6 months or weighing less than 7 kg due to potential anticholinergic side effects.
## Dose Adjustments

## Contraindications

## Adverse Effects

* **Common (Dicyclomine):** Dry mouth, blurred vision, urinary retention, constipation, drowsiness, confusion (especially in the elderly).
* **Serious:** Gastrointestinal bleeding/perforation, renal papillary necrosis, hepatic failure, Stevens-Johnson syndrome, anaphylaxis.
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding due to mefenamic acid's effect on platelet function and potential GI irritation.
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **Other NSAIDs:** Additive effects, increasing risk of adverse events.
* **Diuretics, ACE inhibitors, ARBs:** Mefenamic acid can reduce their antihypertensive and diuretic effects and increase the risk of renal impairment.
* **Lithium:** Mefenamic acid can increase lithium levels.
* **Anticholinergics/Antimuscarinics:** Additive anticholinergic effects with dicyclomine.
## Monitoring
* **Renal function:** Particularly in patients with pre-existing renal disease or those on concomitant nephrotoxic agents.
* **Hepatic function:** Monitor liver enzymes, especially with prolonged use.
* **Signs of gastrointestinal bleeding:** Stool guaiac, symptoms of dyspepsia, melena.
* **Signs of anticholinergic effects:** Dry mouth, blurred vision, urinary hesitancy, constipation.
## Clinical Pearls
* Due to the anticholinergic component, caution is advised in the elderly and in patients with conditions exacerbated by anticholinergic therapy.
* Mefenamic acid should be used at the lowest effective dose for the shortest duration necessary to minimize risks.
* Avoid concomitant use with other NSAIDs or aspirin unless specifically indicated and monitored.
* The risk of gastrointestinal bleeding is increased in patients with a history of ulcers or bleeding.
* Hydration is important, especially to mitigate anticholinergic effects like dry mouth and urinary retention.
