# Meftal spas
## Overview
Meftal spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and dicyclomine hydrochloride, an anticholinergic antispasmodic.
## Primary Indications
Meftal spas is indicated for the symptomatic relief of mild to moderate pain associated with smooth muscle spasms, such as in dysmenorrhea and gastrointestinal colic.
## Adult Dosing
* **Typical Dose:** 1 tablet orally every 8 hours as needed.
* **Maximum Dose:** Not to exceed 4 tablets per day. Treatment duration should be limited to the shortest effective period.
## Pediatric Dosing
There is no established pediatric dosing for Meftal spas. Use in children is generally not recommended.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Dose reduction may be necessary, especially in severe impairment. Mefenamic acid is renally excreted.
* **Hepatic Impairment:** Use with caution. Mefenamic acid is metabolized in the liver.
* **Elderly:** Use with caution due to increased risk of adverse effects, particularly gastrointestinal bleeding and renal events.
## Contraindications
* Hypersensitivity to mefenamic acid, dicyclomine, or other NSAIDs.
* Patients with a history of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Severe renal or hepatic impairment.
* Active gastrointestinal bleeding or history of recurrent gastrointestinal ulceration.
* Inflammatory bowel disease.
* Glaucoma (angle-closure).
* Myasthenia gravis.
* Obstructive uropathy or gastrointestinal obstruction.
* Unstable heart failure.
## Adverse Effects
* **Common:** Nausea, vomiting, abdominal pain, diarrhea, constipation, dizziness, drowsiness, blurred vision, dry mouth.
* **Serious:** Gastrointestinal bleeding, ulceration, perforation; renal toxicity; hepatic toxicity; cardiovascular events (MI, stroke); hypersensitivity reactions; exacerbation of glaucoma; urinary retention; confusion.
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **Other NSAIDs and Aspirin:** Increased risk of gastrointestinal adverse effects and bleeding.
* **Diuretics and Antihypertensives:** May reduce efficacy and increase risk of renal toxicity.
* **Lithium:** Increased serum lithium levels.
* **Methotrexate:** Increased methotrexate toxicity.
* **Anticholinergics:** Additive anticholinergic effects.
* **Antacids:** May affect absorption of mefenamic acid.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding (e.g., melena, hematemesis).
* Monitor renal function (serum creatinine, BUN) especially in patients with pre-existing renal disease or those on concomitant nephrotoxic agents.
* Monitor liver function (LFTs) periodically.
* Assess for signs of anticholinergic side effects (e.g., dry mouth, urinary retention, blurred vision, constipation).
## Clinical Pearls
* Meftal spas should be taken with food or milk to minimize gastrointestinal upset.
* Limit duration of use to the shortest period necessary to control symptoms.
* Discontinue use and seek medical attention if signs of bleeding or significant adverse effects occur.
* Due to potential for drowsiness and blurred vision, caution patients regarding driving or operating machinery.
