# Meftal spas
## Overview
Meftal spas is a combination product containing mefenamic acid and dicyclomine. Mefenamic acid is a non-steroidal anti-inflammatory drug (NSAID) that inhibits prostaglandin synthesis. Dicyclomine is an anticholinergic antispasmodic agent.
## Primary Indications
* Short-term treatment of mild to moderate pain associated with menstrual cramps (dysmenorrhea).
## Adult Dosing
* **Mefenamic Acid:** 500 mg every 6 hours as needed. Maximum daily dose: 1500 mg.
* **Dicyclomine:** 20 mg every 6 hours as needed. Maximum daily dose: 80 mg.
*Note: Specific dosing regimens may vary based on local protocol and patient response.*
## Pediatric Dosing
* This combination product is not typically recommended for pediatric patients due to limited safety and efficacy data. Dosing for individual components in children is complex and should be determined by a qualified healthcare provider.
## Dose Adjustments
* **Renal Impairment:** Use with caution. Mefenamic acid is renally excreted. Consult prescribing information for specific guidance.
* **Hepatic Impairment:** Use with caution. Mefenamic acid can affect liver function.
* **Elderly:** Increased risk of adverse effects, particularly gastrointestinal bleeding, renal, and cardiovascular events. Use with caution and at the lowest effective dose.
## Contraindications
* Known hypersensitivity to mefenamic acid, dicyclomine, aspirin, or other NSAIDs.
* Active gastrointestinal ulceration or bleeding.
* Inflammatory bowel disease.
* Severe renal or hepatic impairment.
* Myasthenia gravis.
* Glaucoma.
* Obstructive uropathy or gastrointestinal obstruction.
* Uncontrolled heart failure.
* Concurrent use with other NSAIDs, including aspirin.
* Pregnancy (especially in the third trimester).
* Breastfeeding.
## Adverse Effects
* **Common:** Nausea, vomiting, abdominal pain, diarrhea, constipation, dizziness, drowsiness, dry mouth.
* **Serious:** Gastrointestinal bleeding, ulceration, perforation; cardiovascular thrombotic events (MI, stroke); renal failure; hepatic failure; hypersensitivity reactions (including anaphylaxis); blurred vision; urinary retention; exacerbation of glaucoma.
## Key Drug Interactions
* **Other NSAIDs (including aspirin):** Increased risk of gastrointestinal adverse effects and renal toxicity.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **SSRIs/SNRIs:** Increased risk of gastrointestinal bleeding.
* **Diuretics, ACE inhibitors, ARBs:** Reduced antihypertensive effect and increased risk of renal impairment.
* **Anticholinergic agents:** Additive anticholinergic effects.
* **Digoxin:** May increase digoxin levels.
## Monitoring
* **Renal function:** Monitor periodically, especially in patients with pre-existing renal disease or those on long-term therapy.
* **Hepatic function:** Monitor periodically, especially in patients with pre-existing hepatic disease.
* **Gastrointestinal symptoms:** Educate patients on signs of bleeding (e.g., black stools, hematemesis) and to report them immediately.
* **Blood pressure:** Monitor for potential increases, especially with NSAID use.
* **Signs of anticholinergic toxicity:** Blurred vision, dry mouth, urinary retention, constipation.
## Clinical Pearls
* Meftal spas should be used for the shortest duration necessary to control symptoms.
* Take with food or milk to minimize gastrointestinal upset.
* Avoid concurrent use of alcohol, which can increase the risk of gastrointestinal bleeding.
* Patients with asthma, cardiovascular disease, or hypertension should use NSAIDs with caution.
* Dicyclomine's anticholinergic effects can cause significant side effects, particularly in the elderly.
