# Meftal spas
## Overview
Meftal spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and hyoscine butylbromide, an antispasmodic agent.
## Primary Indications
* Symptomatic relief of abdominal pain and cramps associated with spasms of the gastrointestinal tract, particularly in conditions like irritable bowel syndrome.
* Management of dysmenorrhea (menstrual cramps).
## Adult Dosing
* **Mefenamic Acid:** Typically 250 mg to 500 mg orally every 6 to 8 hours as needed. Do not exceed 1500 mg per day for short-term use.
* **Hyoscine Butylbromide:** Typically 10 mg to 20 mg orally up to 4 times daily.

## Pediatric Dosing
There is **limited established pediatric dosing** for this combination product. ## Dose Adjustments

## Key Drug Interactions
* **Mefenamic Acid:**
* Anticoagulants (e.g., warfarin): Increased risk of bleeding.
* Corticosteroids: Increased risk of gastrointestinal ulceration and bleeding.
* SSRIs: Increased risk of gastrointestinal bleeding.
* ACE inhibitors, ARBs, diuretics: Reduced antihypertensive effect and increased risk of renal impairment.
* Lithium: Increased lithium levels.
* Methotrexate: Increased methotrexate toxicity.
* **Hyoscine Butylbromide:** Additive anticholinergic effects with other drugs possessing anticholinergic properties (e.g., tricyclic antidepressants, antihistamines, antipsychotics).
## Monitoring
* **Gastrointestinal:** Monitor for signs and symptoms of bleeding or ulceration.
* **Renal:** Monitor renal function, especially in patients with pre-existing renal disease or those on concomitant nephrotoxic agents.
* **Hepatic:** Monitor liver function tests periodically.
* **Cardiovascular:** Be aware of the increased risk of cardiovascular events with NSAID use.
* **Hydration and Electrolytes:** Especially important if diarrhea is present.
## Clinical Pearls
* Meftal spas should be used for the shortest duration necessary to control symptoms.
* Lowest effective dose should be used.
* Consider gastroprotection (e.g., PPI) in patients at high risk of GI bleeding.
* Concomitant use with other NSAIDs should be avoided.
* Effectiveness and safety in pediatric populations are not well-established.
