# Meftal spas
## Overview
Meftal spas is a combination product containing mefenamic acid, a nonsteroidal anti-inflammatory drug (NSAID), and hyoscine butylbromide, an antispasmodic agent.
## Primary Indications
Meftal spas is indicated for the relief of mild to moderate pain and spasms associated with gastrointestinal conditions, dysmenorrhea, and headache.
## Adult Dosing
The typical adult dose is one tablet orally every 8 hours, as needed. Do not exceed 3 tablets in 24 hours.
## Pediatric Dosing
Specific pediatric dosing for Meftal spas is not established. Mefenamic acid dosing in pediatrics is typically based on weight. Consult pediatric-specific guidelines for mefenamic acid if considering use in children.
## Dose Adjustments
No specific dose adjustments are routinely recommended for renal or hepatic impairment. However, caution is advised in patients with severe renal or hepatic disease.
## Contraindications
* Hypersensitivity to mefenamic acid, hyoscine butylbromide, or any component of the formulation.
* History of asthma, urticaria, or allergic-type reactions after aspirin or other NSAIDs.
* Severe renal, hepatic, or cardiac failure.
* Myasthenia gravis.
* Tachycardia, prostatic hypertrophy with urinary retention, or mechanical stenosis along the gastrointestinal tract.
* Closed-angle glaucoma.
* Concurrent use with other NSAIDs.
## Adverse Effects
Common adverse effects include gastrointestinal upset (nausea, vomiting, diarrhea, constipation), dizziness, drowsiness, headache, and rash. More serious potential adverse effects include gastrointestinal bleeding, perforation, renal toxicity, cardiovascular thrombotic events, and hypersensitivity reactions. Anticholinergic effects of hyoscine butylbromide may include dry mouth, blurred vision, and urinary retention.
## Key Drug Interactions
* **Other NSAIDs and Aspirin:** Increased risk of gastrointestinal bleeding and ulceration.
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal bleeding.
* **Diuretics, ACE inhibitors, Angiotensin II Receptor Blockers:** Reduced antihypertensive effect and increased risk of renal impairment.
* **Lithium:** Increased lithium levels.
* **Methotrexate:** Increased methotrexate toxicity.
* **Anticholinergic Agents:** Additive effects with hyoscine butylbromide.
## Monitoring
Monitor for signs and symptoms of gastrointestinal bleeding. Assess renal function periodically, especially in patients with pre-existing renal disease or those on concomitant nephrotoxic medications. Monitor for signs of cardiovascular events.
## Clinical Pearls
* Meftal spas should be taken with food or milk to minimize gastrointestinal upset.
* Due to the potential for drowsiness and dizziness, patients should exercise caution when operating machinery or driving.
* Use for the shortest duration necessary to control symptoms.
* Avoid concurrent use of alcohol.
