# Mefenamic Acid
## Overview
Mefenamic acid is a nonsteroidal anti-inflammatory drug (NSAID) used for the short-term treatment of mild to moderate pain and for the relief of primary dysmenorrhea. It acts by inhibiting cyclooxygenase (COX) enzymes, thereby reducing prostaglandin synthesis.
## Primary Indications
* Mild to moderate pain
* Primary dysmenorrhea
## Adult Dosing
* **Pain:** 500 mg every 6 hours as needed. Not to exceed 7 days of treatment.
* **Primary Dysmenorrhea:** 500 mg every 6 hours as needed, starting at the onset of pain or bleeding, whichever comes first. Not to exceed 7 days of treatment.
## Pediatric Dosing
Mefenamic acid is generally not recommended for use in children under 14 years of age due to limited data and potential for adverse effects.
## Dose Adjustments
No specific dose adjustments are recommended for hepatic or renal impairment, however, caution is advised. NSAIDs should be used with caution in patients with pre-existing renal disease.
## Contraindications
* Known hypersensitivity to mefenamic acid or other NSAIDs.
* History of asthma, urticaria, or allergic-type reactions after taking aspirin or other NSAIDs.
* Perioperative pain in the setting of coronary artery bypass graft (CABG) surgery.
## Adverse Effects
Common adverse effects include diarrhea, nausea, vomiting, abdominal pain, constipation, and headache. Serious adverse effects may include gastrointestinal bleeding, ulceration, perforation, renal failure, cardiovascular thrombotic events, myocardial infarction, stroke, and hypersensitivity reactions.
## Key Drug Interactions
* **Anticoagulants (e.g., warfarin):** Increased risk of bleeding.
* **Corticosteroids:** Increased risk of gastrointestinal ulceration and bleeding.
* **Other NSAIDs (including aspirin):** Increased risk of adverse effects.
* **Diuretics:** May reduce diuretic efficacy and increase risk of nephrotoxicity.
* **Lithium:** Increased lithium levels and toxicity.
* **Methotrexate:** Increased methotrexate toxicity.
## Monitoring
* Monitor for signs and symptoms of gastrointestinal bleeding (e.g., melena, hematemesis).
* Monitor for signs and symptoms of cardiovascular events (e.g., chest pain, shortness of breath).
* Monitor renal function, especially in patients with pre-existing renal disease.
* Monitor for signs of hypersensitivity reactions.
## Clinical Pearls
* Mefenamic acid is intended for short-term use only.
* Instruct patients to take with food or milk to minimize gastrointestinal upset.
* Advise patients to avoid concurrent use with other NSAIDs or aspirin unless specifically directed by a healthcare provider.
* Discontinue use immediately if any signs of gastrointestinal bleeding or cardiovascular events occur.
