# Magnesium Sulfate
## Overview
Magnesium sulfate is an essential mineral that plays a role in numerous physiological processes, including neuromuscular function, cardiac rhythm, and protein synthesis. It is available for intravenous (IV) and intramuscular (IM) administration.
## Primary Indications
* **Eclampsia and Preeclampsia:** Prevention and treatment of seizures in pregnant women with these conditions.
* **Hypomagnesemia:** Treatment of documented magnesium deficiency.
* **Torsades de Pointes:** Treatment of polymorphic ventricular tachycardia associated with a prolonged QT interval.
* **Bronchospasm:** Adjunctive therapy for severe, acute asthma or COPD exacerbations refractory to standard treatment.
## Adult Dosing
* **Eclampsia/Preeclampsia:**
* Loading dose: 4-6 grams IV infused over 5-20 minutes.
* Maintenance dose: 1-2 grams per hour IV infusion.
* Alternatively, IM: 5 grams in each buttock (10 grams total) every 4 hours.
* **Hypomagnesemia:**
* Severe deficiency: 4-6 grams IV infused over 10-60 minutes, followed by maintenance infusion of 1-2 grams per hour as needed to maintain serum magnesium levels.
* Less severe deficiency: 1-2 grams IV infused over 5-60 minutes, or 1 gram IM every 4 hours for 3-4 doses.

* **Torsades de Pointes:**
* 1-2 grams IV bolus, followed by infusion of 0.5-1 gram per hour as needed.
* **Bronchospasm (Adjunctive):**
* 1-2 grams IV infused over 15-20 minutes.
## Pediatric Dosing
* **Eclampsia/Preeclampsia:** Generally follows adult dosing, but caution and careful monitoring are advised.
* **Hypomagnesemia:** Dosing is highly variable and depends on age, weight, and severity of deficiency. Typically ranges from 20-50 mg/kg/dose (elemental magnesium) IV or IM. Consult specialized pediatric resources or local protocol.
* **Bronchospasm (Adjunctive):** 25-50 mg/kg IV infused over 15-20 minutes, maximum dose 2 grams.
## Dose Adjustments
No dose adjustment is typically required for hepatic impairment. In severe renal impairment, the dose and infusion rate should be reduced and magnesium levels closely monitored to prevent accumulation.
## Contraindications
* Hypermagnesemia.
* Heart block or myocardial damage (IV administration).
* Severe renal impairment (caution advised).
## Adverse Effects
* **Cardiovascular:** Hypotension, flushing, electrocardiographic changes, cardiac arrest.
* **Neuromuscular:** Drowsiness, decreased reflexes, muscle weakness, respiratory depression, paralysis.
* **Gastrointestinal:** Diarrhea (oral forms), nausea, vomiting.
* **Other:** Warmth, sweating, allergic reactions.
## Key Drug Interactions
* **Neuromuscular Blockers:** May potentiate neuromuscular blockade.
* **Calcium:** Concurrent administration may antagonize the effects of magnesium.
* **Digitalis:** IV magnesium can cause digitalis toxicity in patients with digitalis excess.
## Monitoring
* **Serum Magnesium Levels:** Monitor frequently, especially during IV infusions and in patients with renal impairment. Therapeutic levels for hypomagnesemia are typically 1.7-2.2 mmol/L (4-6 mg/dL).
* **Renal Function:** Assess kidney function.
* **Vital Signs:** Blood pressure, heart rate, respiratory rate.
* **Deep Tendon Reflexes:** Assess for loss of reflexes, which indicates hypermagnesemia.
* **Urine Output:** Monitor for adequate urine output, especially in patients receiving high doses.
* **Electrocardiogram (ECG):** Monitor for changes, particularly in patients with cardiac conditions or receiving high doses.
## Clinical Pearls
* Magnesium sulfate administration can cause flushing and a sensation of warmth.
* Rapid IV infusion can lead to hypotension and cardiac arrhythmias.
* In hypomagnesemia, potassium and calcium levels should also be assessed and corrected if necessary, as they can influence magnesium homeostasis.
* Magnesium doses are often expressed as elemental magnesium or as magnesium sulfate heptahydrate. Be aware of the conversion: 1 gram of magnesium sulfate heptahydrate contains approximately 0.167 grams (or 167 mg) of elemental magnesium.
