## Liquid Paraffin
### Overview
Liquid paraffin is a mineral oil used as a laxative. It lubricates the intestinal tract, softening the stool and easing defecation.
### Primary Indications
* Constipation
### Adult Dosing
* **Oral:** 10-45 mL once daily, usually at bedtime.
* **Maximum:** Not explicitly defined, but doses exceeding 45 mL are generally not recommended.
### Pediatric Dosing
* **Oral:** Dosing varies by age and weight. A common starting point is 5-15 mL once daily.
* **Children 6-12 years:** 5-15 mL once daily.
* **Children 1-5 years:** 5-10 mL once daily.
* **Infants <1 year:** Generally not recommended without medical supervision. If used, typically 2.5-5 mL once daily.
* **Note:** Pediatric dosing should be guided by specific product labeling or institutional protocols.
### Dose Adjustments
* No specific dose adjustments are typically required for hepatic or renal impairment.
### Contraindications
* Appendicitis or symptoms of appendicitis.
* Intestinal obstruction or perforation.
* Inflammatory bowel disease (e.g., Crohn's disease, ulcerative colitis).
* Abdominal pain of unknown origin.
* Difficulty swallowing, as aspiration can lead to lipoid pneumonia.
### Adverse Effects
* Anal leakage or soiling.
* Abdominal cramping or discomfort.
* Nausea and vomiting.
* Aspiration, leading to lipoid pneumonia (risk is higher in individuals with impaired swallowing reflexes, or when administered in recumbent positions).
* Malabsorption of fat-soluble vitamins (A, D, E, K) with chronic, high-dose use.
### Key Drug Interactions
* **Fat-soluble vitamins (A, D, E, K):** Reduced absorption with chronic use. Administer vitamins at least 2 hours apart from liquid paraffin.
* **Docusate:** Concurrent use may increase the absorption of mineral oil.
### Monitoring
* Bowel movements (frequency, consistency).
* Signs of abdominal obstruction or inflammation.
* For chronic use: Monitor for signs of fat-soluble vitamin deficiency.
### Clinical Pearls
* Administer at bedtime to minimize daytime disruption and leakage.
* Advise patients to remain in an upright position during and after administration to reduce aspiration risk.
* Avoid use in patients with dysphagia or a risk of aspiration.
* Long-term, daily use should be avoided due to potential for vitamin malabsorption and other complications.
* If symptoms persist or worsen, discontinue use and consult a healthcare provider.
