## Liquid Paraffin
### Overview
Liquid paraffin is a mineral oil used as a lubricant and laxative. It works by coating the stool, making it softer and easier to pass.
### Primary Indications
* Constipation
### Adult Dosing
* **Laxative:** 10 mL to 30 mL orally once daily, usually at bedtime. Doses may vary based on product concentration and individual response.
### Pediatric Dosing
* **Constipation:**
* **1-6 years:** 5 mL orally once daily.
* **6-12 years:** 10 mL orally once daily.
* Dosing for children under 1 year is generally not recommended without physician guidance.
* Maximum daily dose for children should not exceed 10 mL for those under 6 years and 20 mL for those aged 6-12 years.
### Dose Adjustments
* No dose adjustments are typically required for renal or hepatic impairment.
### Contraindications
* Appendicitis or undiagnosed abdominal pain.
* Intestinal obstruction.
* Fecal impaction.
* Individuals with difficulty swallowing, due to risk of lipid pneumonia from aspiration.
### Adverse Effects
* Diarrhea
* Abdominal cramping
* Nausea
* Anal leakage (especially with higher doses)
* Lipid pneumonia (rare, but serious, with chronic aspiration)
### Key Drug Interactions
* **Fat-soluble vitamins (A, D, E, K):** Liquid paraffin can impair the absorption of fat-soluble vitamins with chronic use. Consider separation of doses or vitamin supplementation if long-term use is anticipated.
* **Other medications:** May decrease the absorption of other orally administered medications due to its oily nature and effect on gut motility. Separate administration by at least 2 hours.
### Monitoring
* Bowel movements (frequency, consistency).
* Signs of dehydration or electrolyte imbalance with excessive use.
* Signs of aspiration if risk factors are present.
### Clinical Pearls
* Administer at bedtime to minimize daytime anal leakage.
* Avoid prolonged or excessive use, as it can lead to dependence and vitamin malabsorption.
* Do not administer to individuals who are bedridden, elderly, or have a significant risk of aspiration.
* Use with caution in pregnancy and breastfeeding; consult prescribing information.
